About approval of the Procedure for registration and issue of the passport of the citizen of Ukraine

According to the Laws of Ukraine "About freedom of travel and the free choice of the residence in Ukraine" and "About bases of social protection of homeless citizens and street children", the Regulations on the passport of the citizen of Ukraine approved by the Resolution of the Verkhovna Rada of Ukraine of June 26, 1992 N2503-XII (with changes), and Regulations on State department for nationality, immigrations and registration of physical persons approved by the resolution of the Cabinet of Ministers of Ukraine of June 14, 2002 N844 "About Formation of State Department for Nationality, Immigration and Registration of Physical Persons"

I ORDER:

1. Approve the Procedure for registration and issue of the passport of the citizen of Ukraine (further - the Procedure) it (is applied).

2. To State department for nationality, immigration and registration of physical persons (Cooper V.S.) to organize studying by employees of service for nationality, immigrations and registration of physical persons of the Ministry of Internal Affairs of Ukraine of provisions of the Procedure and to provide its execution.

3. To State department for nationality, immigration and registration of physical persons (Cooper V.S.) to provide provision in accordance with the established procedure of the specified order on state registration in the Ministry of Justice of Ukraine.

4. Declare invalid the order of the Ministry of Internal Affairs of Ukraine of 17.08.94 N 316 "About approval of the Instruction concerning rules and procedure for registration and issue of the passport of the citizen of Ukraine", registered in the Ministry of Justice of Ukraine on September 5, 1994 for N 211/421.

5. To impose control of execution of the order on the deputy minister militia major general Rudik V. M.

Procedure for registration and issue of the passport of the citizen of Ukraine

1. General provisions

1. This Procedure is developed according to the Laws of Ukraine "About freedom of travel and the free choice of the residence in Ukraine" and "About bases of social protection of homeless citizens and street children", the Regulations on the passport of the citizen of Ukraine approved by the Resolution of the Verkhovna Rada of Ukraine of June 26, 1992 N2503-XII (with changes).

2. The passport of the citizen of Ukraine (further - the passport) is issued by territorial subdivisions for nationality, immigration and registration of physical persons of city, district managements (departments) of the Ministry of Internal Affairs (further - territorial subdivisions) in the place of residence to each citizen of Ukraine after achievement of 16-year age, and further exchanges in case of need, issued instead of lost, stolen or spoiled.

Are issued to homeless citizens of the passport in place of registration of their prevailing location.

Issue and exchange of the passport is carried out in a month.

3. For execution of the passport person gives:

the certificate of return of person to accommodation at Ukraine with the corresponding mark in the passport of the citizen of Ukraine for trip abroad issued by employees of territorial authority of service of nationality, immigration and registration of physical persons of head departments, Departments of MIA of Ukraine in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol (further - territorial authorities) or the passport of the citizen of Ukraine for trip abroad - for citizens of Ukraine who constantly lived abroad, after their return to accommodation at Ukraine;

the certificate of release from organization of execution of punishments if before condemnation person had no passport or the passport was not withdrawn from it and was not attached to the personal record;

the certificate of registration issued by the corresponding specialized institution performing accounting of homeless citizens (for homeless citizens).

Originals of documents return to person together with the drawn-up passport.

4. The statement and other documents necessary for registration or exchange of the passport, as a rule move to authorized persons of the relevant housing-and-municipal companies of local government bodies, in the Kiev and Sevastopol city public administrations, housing cooperatives, to associations of co-owners of the apartment house and youth housing complexes which independently perform servicing of houses, authorized persons in the village, settlement councils, housing-and-municipal bodies servicing departmental housing stock, including military camps, other objects and the territories (further - the housing-and-municipal companies).

The citizens living in the houses, apartments belonging to them on the right of personal property which are not serviced by the housing-and-municipal companies file documents directly to territorial subdivisions.

Persons condemned to imprisonment or restriction of freedom submit applications through administration of the relevant organization of execution of punishments.

Documents for registration or exchange of passports to the citizens recognized as incapacitated are drawn up based on the statement of the guardian (custodian).

During reception of an application and other documents authorized persons of the housing-and-municipal companies or employees of territorial subdivisions check correctness of filling of the statement and its compliance to the provided documents about what they make the corresponding entry in column 9 of the statement.

Documents for execution of passports and gluing in of photos move the housing-and-municipal companies in territorial subdivisions within three days.

5. Applications for issue of the passport are considered by employees of territorial subdivisions. During their consideration belonging of person to citizenship of Ukraine in case of need becomes clear. By results of consideration the relevant decision is made about what entry in the corresponding column of the statement is made.

6. Passports are drawn up by employees of territorial subdivisions to whom according to their service duties filling of passports is assigned. Filling is performed according to the filed documents by means of the computer equipment or, in exceptional cases, by hand, legible handwriting, without reducings and corrections. In case of assumption of mistakes or corrections of the passport are liable to destruction according to the procedure, established by Section 9 of the Procedure.
