According to Item 25 of the National plan for accomplishment of the Action plan concerning liberalization of the visa regime by the European Union for Ukraine approved by the Presidential decree of Ukraine of April 22, 2011 No. 494, the subitem 4 of Item 2 of the resolution of the Cabinet of Ministers of Ukraine of July 17, 2009 No. 737 "About measures for ordering of administrative services" Item 3 of the resolution of the Cabinet of Ministers of Ukraine of October 26, 2011 No. 1098 "Some questions of provision by divisions of the Ministry of Internal Affairs and Public migratory service of paid services" I ORDER:
1. Approve:
1.1. The standard of administrative service in registration and issue of the temporary certificate of the citizen of Ukraine which is attached.
1.2. The standard of administrative service in issue of the certificate of registration of person by the citizen of Ukraine which is attached.
1.3. The standard of administrative service in registration and issue of the passport of the citizen of Ukraine for trip abroad which is attached.
1.4. The standard of administrative service in registration and issue of the passport of the citizen of Ukraine for the trip abroad of replacement lost which is attached.
1.5. The standard of administrative service in registration and issue of the travel document of the child for trip abroad which is attached.
1.6. The standard of administrative service in registration and issue of permission to immigration to Ukraine to foreigners or stateless persons which is attached.
1.7. The standard of administrative service in registration and issue of the certificate on permanent or temporary residence which is attached.
1.8. The standard of administrative service in registration and issue of the invitation to physical persons for receipt of visa documents for entry into Ukraine to foreigners and stateless persons which is attached.
1.9. The standard of administrative service in registration and issue of the invitation to legal entities for receipt of visa documents for entry into Ukraine to foreigners and stateless persons which is attached.
1.10. The standard of administrative service in issue of the certificate to the third parties of registration of the place of residence and place of stay of physical persons which is attached.
1.11. The standard of administrative service in entering of data on the second passport of the citizen of Ukraine for trip abroad into the passport of the citizen of Ukraine for trip abroad which is attached.
1.12. The standard of administrative service in entering of information about children into the passport of the citizen of Ukraine for trip abroad which is attached.
1.13. The standard of administrative service in entering of data into the passport of the citizen of Ukraine for trip abroad to citizens who returned on permanent residence to Ukraine which is applied.
1.14. The standard of administrative service in registration to foreigners and stateless persons of prolongation of term of stay in the territory of Ukraine which is applied.
2. To provide to the public migratory service of Ukraine (Kovalchuk M. M.):
2.1. Submission of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.
2.2. Placement of information concerning approval of standards of administrative services which are provided by divisions of the Public migratory service of Ukraine, on the official vebeportal and informing the population through mass media.
3. This order becomes effective from the date of its official publication.
4. Send the order on accessory.

1.1. This Standard determines general information about administrative service in registration and issue of the temporary certificate of the citizen of Ukraine, establishes terms, procedure and the sequence of accomplishment of procedural actions concerning provision by territorial authorities and divisions of the Public migratory service of Ukraine of this administrative service.
1.2. Terms and reducings which are used in the text of this Standard:
GMS - Public migratory service of Ukraine;
Ministry of Internal Affairs - Ministry of Internal Affairs of Ukraine;
territorial authorities of GMS - head departments (managements) of migratory service of Ukraine in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol;
the GMS territorial subdivisions - managements, departments (sectors) of migratory service in areas, areas in the cities, the cities of regional, republican (Autonomous Republic of Crimea) value;
the temporary certificate of the citizen of Ukraine (further - the temporary certificate) is used in the value given in the Law of Ukraine "About citizenship of Ukraine".
1.3. Provision of administrative service is performed according to such regulatory legal acts:
The constitution of Ukraine (with changes);
The law of Ukraine "About citizenship of Ukraine" (with changes);
The presidential decree of Ukraine of 27.03.2001 No. 215 "Questions of the organization of accomplishment of the Law of Ukraine "On citizenship of Ukraine" (with changes);
Presidential decree of Ukraine of 06.04.2011 No. 405 "Questions of the Public migratory service of Ukraine";
the resolution of the Cabinet of Ministers of Ukraine of 17.07.2003 No. 1111 "About approval of Rules of registration and issue of the temporary certificate of the citizen of Ukraine" (with changes);
the resolution of the Cabinet of Ministers of Ukraine of 04.06.2007 No. 795 "About approval of the list of paid services which are provided by divisions of the Ministry of Internal Affairs and the Public migratory service, and the amount of payment for their provision" (with changes);
resolution of the Cabinet of Ministers of Ukraine of 26.10.2011 No. 1098 "Some questions of provision by divisions of the Ministry of Internal Affairs and Public migratory service of paid services";
the resolution of the Cabinet of Ministers of Ukraine of 17.07.2009 No. 737 "About measures for ordering of administrative services" (with changes);
the order of the Ministry of Internal Affairs of Ukraine and the Ministry of Foreign Affairs of Ukraine of 25.12.2003 No. 1598/260 "About approval of forms of the statement questionnaire for execution of the temporary certificate of the citizen of Ukraine and the magazine of their accounting", registered in the Ministry of Justice of Ukraine 23.01.2004 for No. 105/8704 (with changes) (дальшеПриказ No. 1598/260).
1.4. The administrative service is provided by the relevant territorial authority of GMS or the GMS territorial subdivision (further - territorial authority or division of GMS) on the place of residence (registration) of person which petitions for registration and issue of the temporary certificate of the citizen of Ukraine.
Receivers of administrative service are physical persons which reached sixteen-year age, acquired citizenship of Ukraine and undertook the obligation to stop foreign citizenship within two years from the moment of acquisition of citizenship of Ukraine and addressed to the relevant territorial authority or division of GMS with the written application about provision of administrative service.
3.1. For receipt of administrative service in registration and issue of the temporary certificate of the citizen of Ukraine the physical person submits to the relevant territorial authority or division of GMS personally the statement in which the information about the applicant is specified: surname, name, middle name, place of residence, phone.
Such documents are enclosed to the application:
the payment document (receipt) with mark of bank, department of mail service or code of the performed operation on payment of cost of this administrative service;
the statement questionnaire for execution of the temporary certificate of the citizen of Ukraine which form is approved by the Order No. 1598/260;
certificate of registration of person by the citizen of Ukraine;
2 photographs of 35 x 45 mm in size;
persons who constantly live in Ukraine submit the copy of registration number of accounting card of the taxpayer, except physical persons which because of the religious beliefs refuse adoption of registration number of accounting card of the taxpayer and notified on it relevant organ of the State Tax Service and have mark in the passport.
Children aged from 16 up to 18 years can independently transfer to the GMS territorial subdivisions documents for execution of the temporary certificate by it.
3.2. Documents are accepted on condition of presentation of the available passport document or the document which replaces it and proves the applicant's identity. In case of need the certified transfer of the text of the document into Ukrainian moves.
3.3. All documents connected with execution of the temporary certificate join the statement.
3.4. The statement and documents which join it are accepted according to the inventory which copy is issued to the applicant with mark about acceptance date of documents the relevant territorial authority or division of GMS and the signature of the official of territorial authority or division of GMS.
3.5. Application forms are placed at the information stand in territorial authority or division of GMS.
3.6. The materials accepted by the official GMS are registered in register.
3.7. By provision of administrative service the additional accompanying services are not provided.
4.1. Receiver of administrative service:
submits to personally relevant territorial authority or division of GMS the documents specified in Item 3.1 of the Section ІІІ this Standard. In case of need provides further explanations or does amendments;
having arrived for receipt of result of administrative service to the relevant territorial authority or division of GMS, checks correctness of the data on itself entered in the form of the document by the official GMS and appends the signature in this document;
receives the drawn-up temporary certificate from the official of territorial authority or division of GMS, at the same time having put down the signature about it in register.
4.2. Official of territorial authority or division of GMS:
checks completeness of the documents filed by the receiver of administrative service specified in Item 3.1 of the Section ІІІ this Standard;
in case of need suggests the applicant to offer further explanations or to make amendments;
registers the accepted materials in register;
issues the copy of the list of documents which are enclosed to the application;
performs consideration of the statement questionnaire for execution of the temporary certificate of the citizen of Ukraine with observance of requirements of the resolution of the Cabinet of Ministers of Ukraine of 17.07.2003 No. 1111 "About approval of Rules of registration and issue of the temporary certificate of the citizen of Ukraine";
by results of consideration of the application the head (or his deputy) the relevant territorial authority or division of GMS makes the decision concerning registration and issue of the temporary certificate or reasonable refusal in its issue;
performs issue of the temporary certificate personally to the applicant. At the same time person who receives the temporary certificate in register appends the signature with indication of surname and the date of receipt. During issue of the temporary certificate to person the requirement of Item 2 parts one of article 19 of the Law of Ukraine "About citizenship of Ukraine" according to which the foreigner who acquired citizenship of Ukraine and did not give according to the procedure provided by Article part five 8, Item 2 parts two of Article 9 and part two of article 10 of the Law of Ukraine "About citizenship of Ukraine", the document on the termination of foreign citizenship or the declaration on refusal of it, loses citizenship of Ukraine is explained.
If the drawn-up temporary certificate is not received within 30 days after giving by the receiver of administrative service of the statement, within 10 working days after the termination of the specified term the corresponding message is sent it. In case of decision making about refusal in issue of the temporary certificate of it the applicant has enough in writing.
5.1. The territorial authority or division of GMS makes the decision on issue of the temporary certificate or on refusal in its issue in time not later, than ten working days from receipt date of the statement and the documents enclosed to the application.
Applications are considered only according to the procedure of priority according to receipt date.
5.2. The message on decision making on issue of the temporary certificate or on refusal in issue of the temporary certificate is sent issued) to the applicant in writing within three working days from acceptance date of the relevant decision. In the decision on refusal in issue of the temporary certificate the bases of such refusal are specified.
The territorial authority or division of GMS draws up the temporary certificate within three working days from the date of receipt of the document confirming introduction of payment for issue of the temporary certificate.
The official of territorial authority or division of GMS does mark about acceptance date of the documents confirming introduction by the applicant of payment for issue of the temporary certificate for copies of the inventory which was issued to the applicant in case of reception of an application.
5.3. If the applicant within thirty calendar days from the date of the direction to him of the message on decision making on registration and issue of the temporary certificate did not file the document confirming introduction of payment for issue of the temporary certificate or did not address to territorial authority or division of GMS for receipt of the drawn-up temporary certificate, the territorial authority or division of GMS has the right to cancel the decision on issue of the temporary certificate or to make the decision on the termination of consideration of the application on issue of the temporary certificate.
6.1. The territorial authority or division of GMS refuses provision of administrative service in case of filing of application with violation of the requirements specified in Item 3.1 of the Section ІІІ this Standard.
6.2. The decision on refusal shall contain the refusal bases in provision of the specified administrative service. This decision can be appealed in higher authorized body, and also in administrative court.
7.1. Resulting effect of provision of administrative service is issue to the applicant of the temporary certificate.
7.2. The temporary certificate is issued for a period of up to two years.
7.3. The temporary certificate is given in the relevant territorial authority or division of GMS along with the document which confirms the termination by person of foreign citizenship, or with the declaration on refusal of foreign citizenship, and the passport of the citizen of Ukraine is issued to the applicant.
8.1. The administrative service is paid. Its payment is performed according to the sizes determined by the list of paid services which are provided by divisions of the Ministry of Internal Affairs and the Public migratory service, and the amount of payment for their provision approved by the resolution of the Cabinet of Ministers of Ukraine of 04.06.2007 No. 795 (with changes).
8.2. Acceptance of payment for administrative service directly by employees of divisions of GMS is forbidden.
Requirements to officials which directly provide provision of administrative service are established by the legislation of Ukraine on public service.
The documents necessary for provision of administrative service move directly physical person or his representative in territorial authority or division of GMS according to place of registration (place of residence).
Receiving and delivery of documents are performed according to the approved schedule of territorial authority or division of GMS according to the schedule of the working day.
The administrative service is provided on measure of receipt of statements and documents to it in the terms determined by the legislation.
Information on regulatory legal acts which settle procedure for provision of administrative service the procedure for provision, operating mode, this Standard and other accompanying information are posted on the GMS (www.dmsu.gov.ua) website. Territorial authorities and divisions of GMS provide placement at information stands in places of provision of administrative service of exhaustive information concerning its provision, the list of necessary documents and their samples, details for implementation of payment of administrative service and the location of the nearest banks or departments of mail service.
The administrative service is provided under general conditions.
Registration of claims is carried out according to the Law of Ukraine "About addresses of citizens" and is considered in terms according to the current legislation. The claim is submitted in higher authorized body or on personal acceptance of the head of territorial authority or division of GMS.
