WE ORDER:

1. Approve the enclosed Instruction "About procedure for acceptance, registration, delivery, personification, issue and accounting of the all-civil passport of the citizen of the Kyrgyz Republic".

2. To the Ministry of Internal Affairs of the Kyrgyz Republic, the Ministry of Foreign Affairs of the Kyrgyz Republic, the Ministry of Transport and Communications of the Kyrgyz Republic and the State agency of information resources and technologies under the Government of the Kyrgyz Republic this order to inform the relevant divisions.

3. To impose control of execution of this order on Department of passport and visa control of the Ministry of Internal Affairs of the Kyrgyz Republic, Department of consular service of the Ministry of Foreign Affairs of the Kyrgyz Republic, Management of communication and informatization of the Ministry of Transport and Communications of the Kyrgyz Republic and department of personification and state procurements of the State agency of information resources and technologies under the Government of the Kyrgyz Republic.

4. This order becomes effective after state registration in the Ministry of Justice of the Kyrgyz Republic and the subsequent official publication in the Appendix "Regulations of the Government of the Kyrgyz Republic, Ministries, State Committees, Administrative Departments, Social Fund of the Kyrgyz Republic, National Bank of the Kyrgyz Republic and Local Self-government of the City of Bishkek" to the newspaper of "Erkin-limited liability partnership".

1. General provisions

1.1. This Instruction "About procedure for acceptance, registration, personification, issue and accounting of the all-civil passport of the citizen of the Kyrgyz Republic" (further - the Instruction) is developed according to the Provision "About the All-civil Passport of the Citizen of the Kyrgyz Republic of Sample of 2006" approved by the order of the Government of the Kyrgyz Republic of November 10, 2006 No. 787 "About approval of the Provision, the description and sample of the all-civil passport of the citizen of the Kyrgyz Republic of sample of 2006" and in pursuance of the order of the Government of the Kyrgyz Republic of November 22, 2006 No. 813 "About providing citizens of the Kyrgyz Republic with national passports and the course of accomplishment of the order of the Government of the Kyrgyz Republic of March 20, 2006 with No. 174 "About the course of accomplishment of decisions of the Government of the Kyrgyz Republic and emergency measures on stabilization of situation with providing citizens of the Kyrgyz Republic with national passports".

1.2. The instruction determines procedure for acceptance, registration, delivery and verification of documents for personification of OGP of sample of 2006, and also issue to his citizens in the territory of the Kyrgyz Republic and being beyond its limits.

1.3. Acceptance and document creation of citizens of the Kyrgyz Republic for personification of OGP, and also perform their issue:

- in the territory of the Kyrgyz Republic - territorial passport subdivisions of Department of passport and visa control of the Ministry of Internal Affairs of the Kyrgyz Republic (further - passport divisions);

- outside the Kyrgyz Republic - diplomatic representations or consular establishments of the Kyrgyz Republic.

1.4. Delivery of documents for personification of OGP and the personified OGP in the territory of the Kyrgyz Republic is performed by the Public courier service and the State company "Spetssvyaz" of the Ministry of Transport and Communications of the Kyrgyz Republic (further - GFS, GP "Spetssvyaz" respectively), and beyond its limits - the Ministry of Foreign Affairs of the Kyrgyz Republic.

1.5. Ensuring personification of OGP, forming and maintaining the information database of the personified OGP is performed by the State agency of information resources and technologies under the Government of the Kyrgyz Republic (further - Gosinformresursov).

2. Procedure for acceptance, document creation and the procedure of issue of OGP to the citizens of the Kyrgyz Republic living in the territory of the Kyrgyz Republic

2.1. For obtaining, exchange of OGP the citizen of the Kyrgyz Republic represents in place of registration to passport division the following documents:

2.1.1. For the citizens who reached 16-year age:

- statement for issue of the passport, form No. 1 (Appendix No. 1);

- accounting card on the citizen of the Kyrgyz Republic who received the passport for trip abroad (Appendix No. 2);

- the statement for issue of the passport for trip abroad in duplicate (Appendix No. 3);

- the passport of the citizen of the Kyrgyz Republic of sample of 2004 or the certificate of birth if the passport of the citizen of the Kyrgyz Republic of sample of 2004 was not issued;

- assigned certificate;

- the certificate of social protection (IPN) issued by Social fund if the passport of the citizen of the Kyrgyz Republic of sample of 2004 was not issued;

- 5 (five) color photos on white background of 4х6 cm in size (requirements to the photo are given in Appendix No. 12);

- the receipt on payment for the application blank on receipt (exchange) of the passport;

- the receipt on payment of cost for the personified OGP;

- receipts on payment of the state fee for issue of OGP;

- the application form on the receipt (exchange) of the passport which is filled in and signed by the applicant (further - the statement) (Appendix No. 4).

If the applicant, along with the statement on receipt of OGP, submits the application for receipt of the passport of the citizen of the Kyrgyz Republic of sample of 2004, in order to avoid return of documents, on the application blank on receipt of OGP in the right upper corner over the photo of the applicant the mark of the following content becomes: "The application is at the same time submitted for receipt of the passport of the citizen of the Kyrgyz Republic of sample of 2004".

2.1.2. For the citizens for the first time receiving OGP:

- statement for issue of the passport, form No. 1;

- accounting card on the citizen of the Kyrgyz Republic who received the passport for trip abroad;

- the statement for issue of the passport for trip abroad in duplicate;

- passport of the citizen of the Kyrgyz Republic of sample of 1994 or 2004;

- the assigned certificate, the military ID or the document replacing it for persons liable for call-up;

- the certificate of social protection (IPN) issued by Social fund if the passport of the citizen of the Kyrgyz Republic of sample of 2004 was not issued;

- 5 (five) color photos on white background of 4х6 cm in size;

- the receipt on payment for the application blank on receipt (exchange) of the passport;

- the receipt on payment of cost for the personified OGP;

- receipts on payment of the state fee for issue of OGP;

- the application form on the receipt (exchange) of the passport which is filled in and signed by the applicant.

2.1.3. For the citizens performing exchange of OGP (in connection with filling of pages for marks, expiration of the passport, change of personal data and spoil):

- the all-civil passport which is subject to exchange;

- statement for issue of the passport, form No. 1;

- accounting card on the citizen of the Kyrgyz Republic who received the passport for trip abroad;

- the statement for issue of the passport for trip abroad in duplicate;

- 5 (five) color photos on white background of 4х6 cm in size;

- the document certifying the fact of change of surname, name, middle name in case of change of pesonalny data;

- the receipt on payment for the application blank on receipt (exchange) of the passport;

- the receipt on payment of cost for the personified OGP;

- receipts on payment of the state fee for issue of OGP;

- the application form on the receipt (exchange) of the passport which is filled in and signed by the applicant.

2.1.4. For the citizens receiving OGP in connection with loss/theft:

- statement for issue of the passport, form No. 1;

- accounting card on the citizen of the Kyrgyz Republic who received the passport for trip abroad;

- the statement for issue of the passport for trip abroad in duplicate;

- conclusion of law-enforcement bodies about the fact of loss/theft of the passport;

- the military ID or the document replacing it for persons liable for call-up;

- certificate of birth;

- 5 (five) color photos on white background of 4х6 cm in size;

- the document certifying the fact of change of surname, name, middle name in case of change of pesonalny data;

- the receipt on payment for the application blank on receipt (exchange) of the passport;

- the receipt on payment of cost for the personified OGP;

- receipts on payment of the state fee for issue of OGP;

- the application form on the receipt (exchange) of the passport which is filled in and signed by the applicant.

2.1.5. For the citizens who did not reach 16-year age:

- statement for issue of the passport, form No. 1;

- accounting card on the citizen of the Kyrgyz Republic who received the passport for trip abroad;

- the statement for issue of the passport for trip abroad in duplicate;

- the application form which is filled in and signed by his legal representative;

- the certificate of birth or the statement from the birth record;

- passports of parents (legal representatives) or one of parents (in case of lack of the second) who return after reconciliation of documents;
