I ORDER:

1. Approve the Approval procedure, the room confirming compliance for the organization and carrying out gamblings in gambling institution to the requirements established by the Law of Ukraine "About state regulation of activities for the organization and carrying out gamblings" which is attached.

2. Declare invalid the decision of the Commission on regulation of gamblings and lotteries of December 17, 2020 No. 26 "About approval of the Procedure for issue to the owner of the room of the permission confirming the room compliance for the organization and carrying out gamblings in gambling institution to the requirements established by the Law of Ukraine "About state regulation of activities for the organization and carrying out gamblings" registered in the Ministry of Justice of Ukraine on January 15, 2021 at No. 65/35687.

3. - to the head of expert group on development of legal regime Action of City and strategic partnership of management of development of the IT industry Dobronravova A. V. to provide to the deputy director of management in the procedure established by the legislation submission of this order on state registration in the Ministry of Justice of Ukraine.

1. Pleysiti's agency issues permission to the owner of the room (further - the applicant) according to the application for issue of the permission confirming room compliance for the organization and carrying out gamblings in gambling institution to the requirements established by the Law of Ukraine "About state regulation of activities for the organization and carrying out gamblings" (further - the statement) which is submitted personally or through person authorized by it.

4. The statement is stated in any form, signed by the applicant or person authorized by it with own hand or with use of the digital signature with observance of requirements of the legislation in spheres of electronic document management, electronic identification and electronic confidential services, and shall contain:

1) information about the applicant:

for the legal entity - the name of the legal entity, the location, identification code of the legal entity, surname, own name and middle name (in the presence) the head, phone number, the e-mail address;
