1. According to the procedure of representation and document creation for purpose (recalculation) of pensions according to the Law of Ukraine "About obligatory national pension insurance", No. 22-1, approved by the resolution of board of the Pension fund of Ukraine of November 25, 2005 registered in the Ministry of Justice of Ukraine on December 27, 2005 for No. 1566/11846 (in edition of the resolution of board of the Pension fund of Ukraine of July 7, 2014 No. 13-1):
The Section I to state 1) in the following edition:
"I. Request for appointment (recalculation), pension payment
The statement for appointment, recalculation of pension, recovery, transfer from one type of pension into another (The statement for appointment/recalculation of pension - appendix 1); the statement for the termination of transfer of pension into the account of the pensioner in bank and receipt of pension in the place of the actual accommodation, pension payment continuation by proxy, payment of part of pension on disabled members of the family of person which is on complete state providing, pension payment in six months ahead in connection with departure on the permanent residence abroad, payment transfer of pension at the new place of residence (The statement for pension payment - appendix 2); the statement for employment (dismissal), the beginning (termination) of the activities connected with income acquisition which is accrual base of single fee on obligatory national social insurance (appendix 3); the statement for payment to the half-received pension in connection with the death of the pensioner (appendix 4) is represented the applicant in territorial authority of the Pension fund of Ukraine (further - body which grants pension) through the structural division performing acceptance and servicing of persons (further - the service center).
