About modification of the Procedure for accounting about use of forms of certificates on state registration of acts of civil status, and also their storage

1. Bring in the Procedure for accounting about use of forms of certificates on state registration of acts of civil status, and also their storage approved by the order of the Ministry of Justice of Ukraine of October 29, 2012 No. 1578/5, registered in the Ministry of Justice of Ukraine on November 02, 2012 for 1845/22157, such changes:

1.1. ІІ to state Item 2.1 of the Section in such edition:

"2.1. The public registration service of Ukraine provides departments of state registration of acts of civil status of head departments of justice of the Ministry of Justice of Ukraine in the Autonomous Republic of Crimea, in areas, the cities of Kiev and Sevastopol (further - department of state registration of acts of civil status of head department of justice) with three-year inventory of forms of certificates.

Annually Public registration service of Ukraine generalizes reports of departments of state registration of acts of civil status of head departments of justice based on which constitutes calculations and the order register for production and issue of necessary number of the corresponding forms of certificates.

In the procedure established by the legislation the Public registration service of Ukraine conducts procurement of forms of certificates and sends necessary documents to the company which produces forms of certificates (further - the Company).".
