of 20.10.2003 No. 494 "About approval of the Accounting treatment for the income of physical persons in gambling institutions and personification of such faces", registered in the Ministry of Justice of Ukraine 06.11.2003 for No. 1016/8337;
of 29.10.2003 No. 518 "About approval of the Procedure for application of subitem 9.6.9 of Item 9.6 of article 9 of the Law of Ukraine" About tax on the income of physical persons", registered in the Ministry of Justice of Ukraine 10.11.2003 for No. 1024/8345;
of 04.12.2003 No. 586 "About approval of form of tax calculation of the resident paying dividends to taxpayers from the income of physical persons", registered in the Ministry of Justice of Ukraine 19.01.2004 for No. 70/8669;
of 03.10.2007 No. 560 "About modification of form of Tax calculation of the resident paying dividends to taxpayers from the income of physical persons", registered in the Ministry of Justice of Ukraine 10.10.2007 for No. 1158/14425.
2. Department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.) and Department of the taxation of physical persons of the State Tax Administration of Ukraine (Busarev V. V.) in accordance with the established procedure:
submit this order on state registration to the Ministry of Justice of Ukraine;
provide promulgation of this order.
