According to subitem 9.3.3 of Item 9.3 of article 9 of the Law of Ukraine of May 22, 2003 and being guided N889-1V "About Tax on the Income of Physical Persons" by article 8 of the Law of Ukraine "About the State Tax Service in Ukraine", I ORDER:
1. Approve form of tax calculation of the resident paying dividends to taxpayers from the income of physical persons it (is applied).
2. To the director of the department of methodology Pikhotsky V. F. submit this order to the Ministry of Justice of Ukraine on state registration.
3. To impose control of execution of this order on the vice-chairman Lekar of S. I.
