I ORDER:

1. Approve form of the statement which is drawn up by results of holding planned (unplanned) action of the state control (inspection) concerning observance by operators of the market of requirements of the legislation on the materials and objects intended for contact with foodstuff which is applied.

2. In accordance with the established procedure to submit to department of state policy in the field of sanitary and phytosanitary measures and food security this order on state registration to the Ministry of Justice of Ukraine.
