1. Approve:

1) the "Monthly Report on the Issued Instructions on Quarantine Objects in the field of Quarantine of Plants" form according to appendix 1 to this order;

2) the "Report on the Revealed Violations when Importing Quarantine Products by the Countries in the field of Quarantine of Plants" form according to appendix 2 to this order;

3) the "Report on Administrative Penalties in the field of Quarantine of Plants" form according to appendix 3 to this order;

4) the "Information on the Carried-out Processings against Quarantine Objects in the field of Quarantine of Plants" form according to appendix 4 to this order;

5) the "Weekly Report on Compliance with Law in the field of Protection of Plants" form according to appendix 5 to this order;

6) form "Summary information on checks of subjects in the field of breeding livestock production and the taken measures to violators of the Law of the Republic of Kazakhstan "About breeding livestock production" according to appendix 6 to this order.

2. To provide to committee of the state inspection in agro-industrial complex of the Ministry of Agriculture of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on Internet resource of the Ministry of Agriculture of the Republic of Kazakhstan after its official publication.

3. To impose control of execution of this order on the supervising vice-Minister of Agriculture of the Republic of Kazakhstan.

4. This order becomes effective after ten calendar days after day of its first official publication.
