According to Item the Cabinet of Ministers of Ukraine decides 8 parts one of article 212 of the Family code of Ukraine:

1. Approve the Procedure for medical examination of persons who showed willingness to adopt or take the child under guardianship/guardianship, to establish foster home, orphanage of family type, to provide service of patronage over the child or to perform mentorship, and persons living with the arranged child on total living space which is applied.

2. Bring the enclosed changes in resolutions of the Cabinet of Ministers of Ukraine.

3. determine that the conclusions issued before entry into force of this resolution on the state of health of person in form according to appendix 3 to the Procedure of activities for adoption and implementation of the supervision of observance of the rights of the adopted children approved by the resolution of the Cabinet of Ministers of Ukraine of October 8, 2008 No. 905 (The Official Bulletin of Ukraine, 2008, No. 79, Art. 2660; 2011, No. 85, the Art. 3110), and conclusions about the state of health of the applicant in form according to appendix 5 to the Procedure for production by guardianship and custody bodies of the activities connected with protection of the rights of the child approved by the resolution of the Cabinet of Ministers of Ukraine of September 24, 2008 No. 866 (The Official Bulletin of Ukraine, 2008, No. 76, Art. 2561), are valid within 12 months from the date of their issue.

4. This resolution becomes effective in one month from the date of its publication.

Procedure for medical examination of persons who showed willingness to adopt or take the child under guardianship/guardianship, to establish foster home, orphanage of family type, to provide service of patronage over the child or to perform mentorship, and persons living with the arranged child on total living space

1. This Procedure determines the procedure of passing of medical examination by persons who showed willingness to adopt or take the child under guardianship/guardianship, to establish foster home, orphanage of family type, to provide service of patronage over the child or to perform mentorship, guardians/custodians, parents tutors, adoptive parents, and also persons living with the arranged child on total living space and frequency of its carrying out.

2. In this Procedure terms are used in the value given in Bases of the legislation of Ukraine on health protection, the Laws of Ukraine "About child care", "About providing organization-legal conditions of social protection of the orphan children and children deprived of parent guardianship", "About social work with families, children and youth", "About prevention and counteraction to domestic violence".

3. Medical examination of persons is performed for determination of their suitability for health reasons to be adoptive parents, guardians/custodians, adoptive parents, parents tutors, foster tutors, assistants to foster tutors, possibility of persons to live with the arranged child on total living space, and also for identification of conditions and restrictions which can bear health hazard and interests of the arranged child.

4. Medical examination is had:

1) persons who showed willingness to adopt the child, to take the child under guardianship/guardianship, to establish foster home, orphanage of family type, to render service of patronage over the child or to perform mentorship;

2) guardians/custodians, adoptive parents, parents tutors, foster tutors, assistants to foster tutors, mentors;

3) the full age persons living on total living space with persons specified in the subitem of 1 this Item (except persons who showed willingness to adopt the child);

4) the children living on total living space with persons specified in subitems 1-3 of this Item (except persons who showed willingness to adopt the child).

5. Guardians/custodians, adoptive parents, parents tutors, foster tutors, assistants to foster tutors, mentors have medical examination every year.

6. Medical examination is performed according to the appeal of person or his legal representative to healthcare institutions and physical persons - entrepreneurs who are registered in the procedure established by the law and obtained the license for the right of implementation of economic activity on medical practice (further - the subjects of managing performing economic activity on medical practice).

Medical examination of juvenile children (aged up to 14 years) is performed in the presence of parents or other legal representatives.

Medical examination of persons with disability or restrictions of daily functioning is carried out in accordance with general practice.

7. Medical examination is performed by the subjects of managing performing economic activity on medical practice, on specialties: therapy, general practice - family medicine, pediatrics.

8. Before passing of medical examination person, parents or other legal representatives of person provide the informed consent on performing medical examination in form No. 003-6/o "The informed voluntary consent of the patient to performing diagnostics, treatment and to carrying out the transaction and anesthesia and to presence or participation of participants of educational process" approved by the order of the Ministry of Health of February 14, 2012 No. 110.

9. During medical examination the doctor providing primary medical care and about which choice by the patient (his legal representative) submits the corresponding declaration according to the legislation (further - the doctor providing primary medical care) inspects and conducts examination of person, issues the direction on carrying out inspections by specialists doctors, and also on carrying out clinical, laboratory, tool trials.
