1. Approve the enclosed Procedure for collection, generalization and submission of information on the revealed cases of the address of dangerous products which did harm and (or) caused damage to life and (or) health of the person, property, the environment, life and (or) health of animals and plants and concerning which measures, and its placements in general information resources of the integrated information system of the Eurasian Economic Union are taken.

2. This Decision becomes effective after 30 calendar days from the date of its official publication.

Procedure for collection, generalization and submission of information on the revealed cases of the address of dangerous products which did harm and (or) caused damage to life and (or) health of the person, property, the environment, life and (or) health of animals and plants and concerning which measures, and its placements in general information resources of the integrated information system of the Eurasian Economic Union are taken

1. This Procedure determines rules of collection, generalization and submission of information on dangerous products which did harm and (or) caused damage to life and (or) health of the person, property, the environment, life and (or) health of animals and plants and concerning which measures for prevention of damnification (including on withdrawal and withdrawal of dangerous products) (further - products which did harm), and its placements in general information resources of the integrated information system of the Eurasian Economic Union are taken (further respectively - information resource, the Union).

This Procedure is developed for the purpose of prevention of the product circulation, products which did harm, included in the Single list concerning which mandatory requirements within the Customs union, No. approved by the Decision of the Commission of the Customs union of January 28, 2011 526, are established but concerning which are not accepted or technical regulations of the Union did not become effective and are not established according to the legislation of state members of the Union (further - state members) mandatory requirements of safety, and also the products which did harm, are not included in the specified Single list.

This Procedure does not extend to products specified in Item 2 of article 1 of the Agreement on procedure for the address within the Eurasian Economic Union of products, requirements to which are not established by technical regulations of the Eurasian Economic Union, and rules of safety of such products of February 3, 2020, and products which are subject sanitary and epidemiologic, veterinary and to phytosanitary quarantine control (supervision).

2. The concepts used in this Procedure are applied in the values defined by the Agreement on the Eurasian Economic Union of May 29, 2014 and the Agreement specified in paragraph three of Item of 1 this Procedure.

3. Collection and generalization of information on the product circulation which did harm are carried out by the bodies of state members authorized on implementation of monitoring of safety of products specified in the paragraph the second Item of 1 this Procedure (further - authorized bodies), according to the legislation of state members.

4. Information on the revealed cases of the damnification connected with the address of dangerous products, and (or) causing damage of life and (or) to health of the person, property, the environment, lives and (or) to health of animals and plants goes authorized bodies to the Eurasian economic commission (further - the Commission) for its placement in information resource no later than 5 working days from the date of identification of case of such address with indication of the following data:

a) date of detection of the product circulation, done harm;

b) the name and (or) designation of products which did harm, its name (in the presence), date of production and expiration date (service life) of products;

c) the name of the manufacturer (authorized by the manufacturer of person, the importer) products which did harm, the location (the address of the legal entity) and the address (addresses) of the place of implementation of activities (if addresses differ) - for the legal entity or surname, name, middle name (in the presence), the residence and the address (addresses) of the place of implementation of activities (if addresses differ) - for the physical person registered as the individual entrepreneur and also phone numbers and e-mail addresses (in the presence);

d) the name of the legal entity or physical person registered as the individual entrepreneur at which products which did harm, the location (the address of the legal entity) and the address (addresses) of the place of implementation of activities are revealed (if addresses differ) - for the legal entity or surname, name, middle name (in the presence) the residence and the address (addresses) of the place of implementation of activities (if addresses differ) - for the physical person registered as the individual entrepreneur;

e) the data on the document on assessment of conformity issued according to the legislation of state member (registration number, term and the status of action) (in the presence);

e) data on the measures taken according to the legislation of state member concerning products which did harm;
