About introduction of amendments to the resolution of the Cabinet of Ministers of Ukraine of June 29, 2021 No. 677
The Cabinet of Ministers of Ukraine decides:
1. Bring in the resolution of the Cabinet of Ministers of Ukraine of June 29, 2021 No. 677 "Some questions of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease" (The Official Bulletin of Ukraine, 2021, No. 54, the Art. 3342, No. 69, the Art. 4383, No. 76, the Art. 4771, No. 95, the Art. 6153) changes which are applied.
2. Determine that the certificates confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus based on medical records about vaccination against the specified disease taking into account booster dose are created from the moment of technical capability.

1. According to the procedure of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease, approved by the specified resolution:
In paragraph nine of Item 2 of the word "or authorized institution of foreign state with which Ukraine signs the international treaty on mutual recognition" to exclude 1);
2) in paragraph four of Item 3 of the word "one dose of odnodozny vaccine and" shall be replaced with words "at least than one dose of odnodozny vaccine at least".
