Bring in the resolution of the Cabinet of Ministers of Ukraine of May 7, 2014 No. 152 "About approval of sample of the form, technical description and the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, destructions of the passport of the citizen of Ukraine for trip abroad, his temporary detention and withdrawal" (The Official Bulletin of Ukraine, 2014, No. 45, the Art. 1188, No. 61, the Art. 1685, No. 97, Art. 2806; 2015, No. 69, the Art. 2276) changes which are applied.

Changes which are made to the Resolution of the Cabinet of Ministers of Ukraine of May 7, 2014 No. 152

1. In the resolution:

The name and introductory part of the resolution to state 1) in the following edition: "About approval of sample of the form, technical description and the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine for trip abroad

According to part two of Article 15 and part two of article 22 of the Law of Ukraine "About the Unified state demographic register and documents confirming citizenship of Ukraine, proving the identity or her special status" the Cabinet of Ministers of Ukraine decides:";

The paragraph the fourth Item 1 to state 2) in the following edition:

"Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine for trip abroad which is applied.";

3) to add the resolution with item 4 of the following content:

"4. Determine that:

adoption of documents for execution of the passport of the citizen of Ukraine for trip abroad which does not contain the contactless electronic medium which sample of the form is approved by this resolution since December 20, 2016 stops;

the passport of the citizen of Ukraine for trip abroad which does not contain the contactless electronic medium, drawn up and issued based on the documents submitted till December 20, 2016 is effective during the term for which it was issued.".

2. To state appendix 1 to the resolution in the following edition:

3. In appendix 2 to the resolution:

To replace 1) in Item of 1 figure "88 x 125" with figures "88 x 125 (+ 0,75)";

The second Item 2 of the word and figure of "ISO/IEC 14443A standard" shall be replaced with words 2) in the paragraph also in figures of "ISO/IEC 14443 standard";

3) in paragraph nine of Item 7 of the word "law-enforcement bodies" shall be replaced with words "bodies of National police";

Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine for trip abroad

GENERAL PART

1. The passport of the citizen of Ukraine for trip abroad (further - the passport for trip abroad) is the identity document confirming citizenship of Ukraine person addressed to whom it is drawn up and grants the right to such person on departure from Ukraine and entry into Ukraine.

The passport is made for trip abroad in the form of book which right fly-leaf contains the contactless electronic medium.

2. Each citizen of Ukraine has the right to the passport for trip abroad.

3. The citizen of Ukraine cannot have more than two passports for trip abroad.

4. The passport for trip abroad is drawn up to persons which did not reach 16-year age for four years, and to persons which reached 16-year age - for ten years.

5. In case of loss or theft of the passport for trip abroad or stolen the new passport for trip abroad is drawn up and issued to person instead of lost.

6. Exchange of the passport for trip abroad is performed in case:

1) changes of information entered in the passport for trip abroad;

2) identifications of mistake in information entered in the passport for trip abroad;

3) the terminations of effective period of the passport for trip abroad;

4) unfitness of the passport for trip abroad for further use.

7. Registration (including instead of lost or stolen), exchange and issue of the passport for trip abroad are performed:

1) to person which reached 16-year age, - based on the statement questionnaire, provided to them personally;

2) to person which did not reach 16-year age, face which is recognized as court it is limited capable or incapacitated, - based on the statement questionnaire of one of parents (adoptive parents), guardians, custodians or other legal representatives (further - legal representatives) in case of registration (including instead of lost or stolen), exchange of the passport for trip abroad in foreign diplomatic organization - based on the statement questionnaire of one of parents / legal representatives and written consent of other parent / legal representatives which is provided in case of submission of documents (the statement from the second parent / legal representatives is not required if he is foreigner or the stateless person). In case of absence of the second parent in case of submission of documents such application is submitted by notarially certified. If parents are not married, registration (including instead of lost or stolen), exchange and issue of the passport for trip abroad is performed based on the statement questionnaire of that from them with whom person lives;

3) to person who cannot independently move in connection with long disorder of health and who needs urgent medical treatment abroad that is confirmed by the medical certificate of the relevant healthcare institution which is drawn up in accordance with the established procedure - based on the statement of the questionnaire of person or his legal representative.

8. The passport for trip abroad is issued:

1) not later than in 20 working days from the date of execution of the statement questionnaire for its obtaining;

2) not later than in seven working days from the date of execution of the statement questionnaire for its urgent obtaining;

3) till three months - in case of execution of the statement questionnaire in foreign diplomatic organization;

4) till three working days - due to the need of urgent trip abroad, connected with compelling need in treatment driving off, departure of person accompanying the seriously ill patient or the death of the relative who lived abroad;

5) till 10 working days - in case of departure on permanent residence abroad the child citizen of Ukraine adopted by foreigners.
