Make the following changes to the "Description of the Certificate on state social insurance" approved by the resolution of the Cabinet of Ministers of the Azerbaijan Republic of March 23, 2002 No. 46 (Collection of the legislation of the Azerbaijan Republic, 2002, No. 3, Article 149; 2005, No. 6, Article 546; 2008, No. 2, Article 114; 2016, No. 6, Article 1221):
1. In the fifth paragraph of the word "State Social Security Fund" shall be replaced with words "The state Social Security Fund under the Ministry of Labour and Social Protection of the population of the Azerbaijan Republic".
2. In the sixth paragraph after the word "Floor" to add with words ", "Address" and "Individual identification number".
