About introduction of amendments to resolutions of Council of Ministers of the Republic of Belarus of April 30, 2010 No. 661 and of February 17, 2012 No. 156

1. Make changes to the following resolutions of Council of Ministers of the Republic of Belarus:

1.1. in Regulations on the procedure for creation of the request for registration and provision of right to use by the name of the place of goods origin approved by the resolution of Council of Ministers of the Republic of Belarus of April 30, 2010 No. 661 (The national register of legal acts of the Republic of Belarus, 2010, No. 109, 5/31767):

1.1.1. in Item 2:

the fifth to state the paragraph in the following edition:

"the request - the request for registration and provision of right to use by the name of the place of goods origin, and also on right to use provision by already registered name of the place of goods origin;";

to exclude the paragraph of the eighth;

1.1.2. in Item 5 to replace the word "Statement" with the word "Request";

1.1.3. in Item 6 "statement" to replace the word with the word to "request";

1.1.4. state Item 7 in the following edition:

"7. Contain in the request:

7.1. the statement for registration and provision of right to use by the name of the place of goods origin or about right to use provision by already registered name of the place of goods origin with indication of:

7.1.1. the information about the applicant (applicants), including:

full name of the legal entity according to the constituent document or surname, own name, middle name (if that is available) physical person, and surname is specified before own name and middle name (if that is available);
