About some questions concerning one-time coupons

4) data on physical persons which business activity exceeded ninety days in year, according to appendix 4 to this order;

5) data on the used and damaged forms of one-time coupons according to appendix 5 to this order;

6) data on remaining balance of one-time coupons according to appendix 6 to this order;

7) data on the amounts transferred into the budget from issue of one-time coupons according to appendix 7 to this order;

8) data on date of the expiration of thirty days on which one-time coupons by physical persons, according to appendix 8 to this order were acquired.
