b) birth date of physical person;

c) identification taxpayer number of physical person;

d) number of the special account;

e) name and location of authorized bank;

e) bank identification code of authorized bank;

g) number of correspondent account (sub-account) of authorized bank (its branch);

h) date of opening of the special account;

i) date of the conclusion of bank account agreement (in the presence).

2. This resolution becomes effective from the date of its official publication.
