1. Form 4 "The statement for state registration of the legal entity - public forming", approved by the order of the Ministry of Justice of Ukraine of November 18, 2016 No. 3268/5, registered in the Ministry of Justice of Ukraine on November 18, 2016 at No. 1500/29630 (in edition of the order of the Ministry of Justice of Ukraine from July 04, 2023 No. 2469/5), to be reworded as follows which is applied.
2. To department of private law (Matsayenko T.) present tax the order on state registration according to the Presidential decree of Ukraine of October 3, 1992 No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".
3. This order becomes effective since November 1, 2025, but not earlier than day of its official publication.
