Based on part (5) article 7 of the Law No. 100/2001 on acts of civil status (repeated publication: The official monitor of the Republic of Moldova, 2025, Art. No. 536-539, 662) DECIDES: the Government

1. Approve samples of documents on civil registration:

1.1. the statement from the certificate of birth, according to appendix No. 1;

1.2. the statement from the marriage certficate, according to appendix No. 2;

1.3. the statement from the certificate on annulment of marriage, according to appendix No. 3;

1.4. the statement from the certificate on change of surname and/or name, according to appendix No. 4;

1.5. the statement from the death certificate, according to appendix No. 5;

1.6. the statement from civil registrations of person, according to appendix No. 6;

1.7. the certificate of civil status, according to appendix No. 7;

1.8. the certificate on matrimonial legal capacity, according to appendix No. 8;

1.9. the multilingual statement from the certificate of birth (issued based on the Convention of International commission on civil status No. 16 on the issue of multilingual archival statements from acts of civil status signed in Vienna on September 8, 1976), according to appendix No. 9;

1.10. the multilingual statement from the marriage certficate (issued based on the Convention of International commission on civil status No. 16 on the issue of multilingual archival statements from acts of civil status signed in Vienna on September 8, 1976), according to appendix No. 10;

1.11. the multilingual statement from the death statement (issued based on the Convention of International commission on civil status No. 16 on the issue of multilingual archival statements from acts of civil status signed in Vienna on September 8, 1976), according to appendix No. 11;

2. Certificates on civil status, their duplicates and statements from acts of civil status issued before entry into force of this resolution are considered valid if:

2.1. the personal data of the owner specified in the act of civil status based on which the document on civil status was issued were not changed after its issue;

2.2. there are no bases for recognition invalid the document on civil status or the act of civil status based on which it was issued.

3. To provide to the agency of the state services issue of documents on civil registration at the request of persons according to the Law No. 100/2001 on acts of civil status.

4. Recognize invalid:

4.2. The order of the Government No. 558/2007 about approval of single samples of forms of civil registrations (The official monitor of the Republic of Moldova, 2007, Art. No. 74-77, 594), with subsequent changes.

5. This resolution becomes effective from the date of publication in the Official monitor of the Republic of Moldova.
