4) the application form of physical person for receipt of the license and (or) appendix to the license, according to appendix 3 to this order;
5) the application form for receipt of the export license of products which are subject to export control according to appendix 3-1 to this order;
6) the application form for receipt of the import license of products which are subject to export control according to appendix 3-2 to this order;
