According to the paragraph of the seventh of part two of article 2 of the Law of Ukraine "About recovery of solvency of the debtor or recognition by his bankrupt" and for the purpose of effective forming and maintaining the single database about the companies concerning which proceeedings about bankruptcy are initiated, I ORDER:
1. Approve forms of provision by the arbitration manager of information necessary for maintaining the single database of the companies concerning which proceeedings about bankruptcy, and the Instruction concerning filling of these forms which are applied are initiated.
2. To submit to State department concerning bankruptcy this order on state registration to the Ministry of Justice of Ukraine.
3. To impose control of execution of this order on the director of State department concerning bankruptcy Bernazyuk Ya. O.
