About modification of item 4 of the application form about purpose of all types of the public assistance, compensations and privileges

According to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423 (with changes), for the purpose of reduction of the regulatory legal act in compliance with the current legislation I ORDER:

1. Make change to item 4 of the application form about purpose of all types of the public assistance, compensations and privileges approved by the order of the Ministry of social policy of Ukraine of April 21, 2015 No. 441, registered in the Ministry of Justice of Ukraine on April 28, 2015 for No. 475/26920, having added it with the new paragraph the twelfth the following content:

the help on leaving to lonely persons who reached 80-year age and according to the conclusion of VKK of healthcare institution need permanent foreign care (except persons with disability owing to war)

2. To management of development of social insurance and provision of pensions (Kudin V. A.) provide submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. This order becomes effective from the date of its official publication.

4. To impose control of execution of this order on the deputy minister Muzychenko V. V.
