According to part two of article 15 of the Law of Ukraine "About state registration of legal entities, physical persons entrepreneurs and public forming" and in connection with adoption of the resolution of the Cabinet of Ministers of Ukraine of August 19, 2020 No. 721 "About modification of Item 1 of the resolution of the Cabinet of Ministers of Ukraine of December 25, 2015." I ORDER to No. 1133:
1. Item 1 of the order of the Ministry of Justice of Ukraine of November 18, 2016 No. 3268/5 "About approval of forms of statements in the field of state registration of legal entities, physical persons entrepreneurs and public forming", registered in the Ministry of Justice of Ukraine on November 18, 2016 for No. 1500/29630, to add with the new subitem of such content:
"10) Statement for establishment of the requirement of notarial assurance / certificate (form 10).".
2. Form 4 "The statement for state registration of the legal entity - public forming" and form 7 "The statement for state registration of public forming without the status of the legal entity", the Ministries of Justice of Ukraine approved by the order of November 18, 2016 No. 3268/5, registered in the Ministry of Justice of Ukraine on November 18, 2016 for No. 1500/29630 (in edition of the order of the Ministry of Justice of Ukraine from May 19, 2020 No. 1716/5), to be reworded as follows which are applied.
