personnel of service of radiation safety and person responsible for radiation safety are appointed from number of persons employed, had special training on radiation safety at the physical person or legal entity having the license by sight of activities "Special training of the personnel responsible for ensuring nuclear and radiation safety";
provision of the copy of the job description responsible for radiation safety is required only for activities for the treatment with radionuclide sources of isotopes of uranium, thorium and plutonium with activity below the minimum significant activity

copies of the following documents: instructions for ensuring nuclear safety when carrying out physical start-up;
instructions for ensuring nuclear safety during the transporting, overload and storage of the fresh and fulfilled fuel;

the actions plan on protection of the personnel and the population against radiation accident and its effects approved by the applicant;
quality assurance programs of safety when implementing of the declared activities;
production schedules of the accomplishment of the declared works determining the main working methods, consecutive procedure for accomplishment of transactions, limits and working conditions

submission of the instruction for ensuring nuclear safety when carrying out physical start-up, the instruction for ensuring nuclear safety during the transporting, overload and storage of the fresh and fulfilled fuel is required only for activities subspecies use of nuclear materials

production schedules of the accomplishment of the declared works determining the main working methods, consecutive procedure for accomplishment of transactions, limits and working conditions;
the actions plan on protection of personnel and the population against radiation accident and its effects; instructions for accounting and control of sources of ionizing radiation

the actions plan on protection of personnel and the population against radiation accident and its effects requires to be represented only objects 1 and 2 of category of radiation hazard; it is not required to provide the copy of the statement from the inventory report of the radioactive materials, devices and installations containing radioactive materials for subspecies of sales activity of the radioactive materials, devices and installations containing radioactive materials and production and conversion of natural uranium
