1. Make additions and changes to the following resolutions of Council of Ministers of the Republic of Belarus:

to add Item 5 after the word "middle name" with words "(if that is available)";
to add Item 22 after the word of "citizens" with words ", in kolkhoz for the periods after 1965";
in Item 26:
in part one shall be replaced with words the word of "employers" "in the form approved by the Ministry of Labour and Social Protection, the issued employers";
third after the word "reference" to add part with the words "in the form approved by the Ministry of Labour and Social Protection";
