1. Make additions and changes to the following presidential decrees of the Republic of Belarus:

state Item 14-1 in the following edition:
"14-1. The physical person can independently limit itself in visit of gambling institutions and participation in gamblings for a period of six months up to three years by personal giving to the organizer of gamblings of the written application with simultaneous presentation of the document proving his identity.
The written application shall contain data on the name of the organizer of gamblings to whom this application is submitted, surname, own name, middle name (if that is available) physical person, data of the document proving his identity (the name of the document, series (in the presence), number, date of issue, the name of the state body which issued the document (if the document is issued by state body of the Republic of Belarus), identification number (if that is available), the official name of foreign state in which the document is issued), the term of refusal of visit of gambling institution and participation in gamblings, date of the statement, the personal signature. The submitted application is not subject to return, except as specified, specified in part three of this Item. Filing of application to any organizer of gamblings is considered as filing of application to all organizers of gamblings in the territory of the Republic of Belarus.
