For the purpose of reduction of procedure for provision of the state service in licensing of activities in the field of use of the sources of ionizing radiation (generating) (except for case if these sources are used for medical activities) in compliance with requirements of the Concept of development of procedures for granting of the state and municipal services in electronic form approved by the order of the Government of the Russian Federation of 25.12.2013 No. 2516-r (The Russian Federation Code, 13.01. 2014, No. 2 (part II), the Art. 155) and in compliance with the changes made by the order of the Government of the Russian Federation of 06.03.2015 No. 197 "About modification of the order of the Government of the Russian Federation of 12.12.2012 No. 1284" (The Russian Federation Code, 16.03. 2015, No. 11, the Art. 1603), the order of the Government of the Russian Federation of 06.03.2015 No. 201 "About introduction of amendments to some acts of the Government of the Russian Federation" (The Russian Federation Code, 16.03. 2015, No. 11, the Art. 1607), and also in connection with the Federal constitutional Law of 21.03.2014 No. 6-FKZ "About acceptance to the Russian Federation of the Republic of Crimea and education as a part of the Russian Federation new subjects - the Republic of Crimea and the federal city of Sevastopol" (The Russian Federation Code, 24.03. 2014, 12, of the Art. 1201) I order to No.:

1. To state the subitem 5 of Item 22 in the following edition:

"5) copies of the documents confirming availability at workers of the license applicant whose activities are directly connected with the sources of ionizing radiation (generating), the higher education or secondary professional education and additional professional education according to programs of advanced training for radiation safety in volume of at least 72 hours".

2. To state the subitem 4 of Item 24 in the following edition:

"4) copies of the documents confirming availability at workers of the licensee whose activities are directly connected with the sources of ionizing radiation (generating), the higher education or secondary professional education and additional professional education according to programs of advanced training for radiation safety in volume of at least 72 hours conforming to requirements and nature of works (services) in the new address.".

3. To state the subitem 4 of Item 25 in the following edition:

"4) copies of the documents confirming availability at workers of the licensee whose activities are directly connected with the sources of ionizing radiation (generating), the higher education or secondary professional education and additional professional education according to programs of advanced training for radiation safety in volume of at least 72 hours conforming to requirements and nature of new works (new services).".

4. Add the Section of Administrative regulations "Provision of information to applicants and ensuring their access to data on provision of the state service" with pct 61.1:

"61.1. In case of receipt of the inquiry sent by the applicant who provided to Rospotrebnadzor (its territorial authority) the statement for provision (renewal) of the license by means of the federal state information system "Single Portal of the State and Municipal Services (Functions)", the notification on the course of provision of the state service goes to single personal account, and also, at the choice of the applicant, to the e-mail address specified to them or phone number.

The notification on the course of provision of service goes no later than one working day after completion of each ministerial procedure.".

- devices of mobile radiotelephone communication;

- the terminal and other devices located in Rospotrebnadzor and its territorial authorities (with technical capabilities), in the multipurpose centers of provision of the state and municipal services;

- Internet.

On completion of ministerial procedure on receipt of provision of the state service by the applicant of result the authorized specialist expert responsible for issue to the applicant of results of provision of the state service suggests the applicant to take part in quality evaluation of the state service provided to it with use of devices of mobile radiotelephone communication.

In case of the consent of the applicant to participation in quality evaluation of the state service provided to it, the authorized specialist expert responsible for issue to the applicant of results of provision of the state service gives the contact information necessary for detection of his opinion on quality of provision of the state service provided to them, to the automated information system "Information and Analytical Monitoring System of Quality of the State Services".

152. If the applicant refused quality evaluation of provision of the state service by means of devices of mobile radiotelephone communication, after receipt of the state service the authorized specialist expert responsible for issue to the applicant of results of provision of the state service informs the applicant on opportunity to use terminal or other device (if technically possible) for quality evaluation of provision of the state service.

Rospotrebnadzor (its territorial authority) provides provision of results of assessment in the automated information system "Information and Analytical Monitoring System of Quality of the State Services".

153. The authorized specialist expert responsible for issue to the applicant of results of provision of the state service shall inform the applicant on opportunity to estimate quality of provision of the state services with use of Internet network by means of the polling module of information system of the monitoring of the state services posted on the official sites of territorial authorities of Rospotrebnadzor, on the specialized website ("Your control") in Internet network, and also in personal account of the single portal of the state and municipal services.".
