1. Declare invalid the order of the State committee of Ukraine concerning regulatory policy and entrepreneurship of 09.06.2004 No. 66 "About approval of application forms and messages which are provided (are sent) to the state registrar", registered in the Ministry of Justice of Ukraine 29.06.2004 for No. 793/9392.

2. (Zerkal O. V.) to submit to department of interaction with authorities this order on state registration according to the Presidential decree of Ukraine of 3.10.92 No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".

3. To the public registration service of Ukraine (Efimenko L. V.) bring this order to permission of the state registrars of executive committees of city councils of the cities of regional value, district, district public administrations in the cities of Kiev and Sevastopol and provide proper accomplishment.

4. This order becomes effective from the date of its official publication.

5. To impose control over the implementation of this order on the director of the department of interaction with authorities Zerkal O. V.
