1. This Instruction about procedure for ledger maintenance of registration of contracts for rendering psychological assistance (further - the book) determines procedure for ledger maintenance.
2. The book shall be numbered, strung together by the licensee and the Minister of Health or the deputy minister of health care authorized by it in accordance with the established procedure is ratified and it is certified by official stamp.
3. In case of availability at the licensee of separate divisions out of the place of its stay on each place of rendering psychological assistance specified in special permission (license) for rendering psychological assistance (further - the license), the licensee keeps the separate book.
