About modification and amendments in the Law "About Assessment of Functionality of the Personality"

Article 1. In Item 3 of part 1 of article 3 of the Law of May 5, 2021 "About assessment of functionality of the person" (further - the Law) the words "determine and" shall be replaced with words No. ZR-195 "determine and/or".
Article 2. To add part 1 of article 7 of the Law with the new offer of the following content: "The individual plan of servicing developed taking into account extent of functional restrictions can be changed (is added) in the cases and procedure established by the Government. In case of change (amendment) of the individual plan of servicing functional assessment is not carried out".
Article 3. State the first offer of Item 5 of part 3 of article 8 of the law in the following edition:
"5) different bodies and the organizations request, if necessary, information necessary for decision making in the field of functionality assessment, including personal data, concerning persons who submitted the application for functionality assessment for the purpose of receipt of information, including personal data.".
Article 4. To exclude the words "it is constituted on the basis of assessment of functionality of the person and" from part 2 of article 10 of the Law.
Article 5. In part 2 and item 4 of part 6 of article 12 of the law to replace the word "guaranteed" with the word "specified", and to replace the word "guarantee" in Item 1 of part 6 with the word "specifying".
Article 6. To add article 12 of the law with part of 10.1 following contents:
