Paternity of the spouse of mother of the child makes sure record about their marriage, notarially certified consent of the spouse of mother of the child to being the written-down father of the child born as a result of use of auxiliary reproductive technologies after death (death) of the spouse of mother of the child and also the document issued by the medical organization performing use of auxiliary reproductive technologies, and confirming use of biological material of the spouse of mother of the child in case of use of auxiliary reproductive technologies which use pregnancy within term was caused, by the stipulated in Item Federal Law of the 5th Article 21 of January 12, 1995 No. 5-FZ "About veterans".
The choice by the father of name and surname of the child is confirmed by notarially certified consent of the spouse of mother of the child to assignment to the child of name and surname or on assignment to the child of name and surname at the choice of the child's mother.".
