1. Establish:
mark form of the blood type on the AB0 and Rh systems, other systems brought by the organization of health care in the passport of the citizen of the Republic of Belarus and the residence permit in the Republic of Belarus (further if other is not established, - mark about blood type), according to appendix 1;
form of the magazine of registration of the marks of blood type on the AB0 and Rh systems, other systems brought by the organization of health care in the passport of the citizen of the Republic of Belarus and the residence permit in the Republic of Belarus (further if other is not established, - the magazine of registration of marks), according to appendix 2.
2. Determine that:
2.1. introduction of mark about blood type is performed:
the organization of health care which is according to the nomenclature of the organizations of health care established by the resolution of the Ministry of Health of the Republic of Belarus of December 5, 2023 No. 183 "About determination of the nomenclature of the organizations of health care and features of the organization of their activities", the hospital, out-patient and polyclinic organization, the organization of service of blood (further - the organization of health care);
by putting down of print of stamp of violet or black color, its filling and assurance with the personal signature of the health care of the official authorized by the head of the organization with violet or black ink (paste);
2.2. the mark about blood type is registered in the magazine of registration of marks in form according to appendix 2;
2.3. the mark about blood type is cancelled in case of detection of the wrong or inexact data by putting down on this mark of print of stamp "Is cancelled" violet or black color;
2.4. introduction of mark about blood type, its registration and cancellation are performed by person specified in paragraph three of subitem 2.1 of this Item.

4. This resolution becomes effective after its official publication.
