1. For receipt of insurance indemnity under the agreement of compulsory insurance the beneficiary submits to the insurer the following documents.

1.1. The written application about payment of insurance indemnity constituted in any form, containing the details allowing to transfer money within the applied forms of clearing settlements if payment of insurance indemnity will be made via bank transfer.

1.2. Identity document of the beneficiary.

1.3. The documents confirming powers and proving the identity of person who is the legal representative or the representative of the beneficiary who is acting on the basis of the power of attorney.

1.4. The document on the taken place event on transport and its circumstances which is drawn up according to rules of transportation of passengers and baggage by the corresponding mode of transport, confirming damnification to the passenger who is constituted by the carrier or person authorized by carrier concerning the victim or several victims (it is represented in case creation of such document is provided by rules of transportation of passengers and baggage the corresponding mode of transport).

1.5. In case of lack of the document provided by subitem 1.4 of this Item, one of the following documents:

the certificate of the road accident in the form approved by the order of the Ministry of Internal Affairs of the Russian Federation of April 1, 2011 No. 154 "About approval of form of the certificate of the road accident", the registered Ministry of Justice of the Russian Federation on May 5, 2011 No. 20671 (The Russian newspaper of May 13, 2011), the protocol and the resolution on the case of administrative offense or determination about refusal in initiation of proceedings about administrative offense (in case of damnification to the victim as a result of the road accident and in case creation of such documents is stipulated by the legislation the Russian Federation);

the resolution on excitement (refusal in excitement) or about suspension of criminal case or about recognition by the victim or the judgment which took legal effect (in case of damnification to the victim as a result of the event which is not connected with the road accident and in case creation of such documents is stipulated by the legislation the Russian Federation).

2. In case of damnification of life of the victim for receipt of insurance indemnity concerning expense recovery for burial the beneficiary submits to the insurer the documents specified in Item of 1 this Instruction and also the following documents.

2.1. Death certificate of the victim.

2.2. The documents confirming the size of the made necessary expenses on burial of the victim.

3. In case of damnification of life of the insurance indemnity which was injured for obtaining in connection with the death of the victim the beneficiary submits to the insurer the documents specified in Item of 1 this Instruction, the death certificate of the victim, and also one of the following documents.

3.1. The document confirming the right of the beneficiary according to the civil legislation of the Russian Federation on indemnification in case of the death of the supporter:

the marriage certficate - in case of the request for insurance payment concerning the spouse who was for date of death dependent on the injured (supporter);

the certificate of birth, adoption of child(children), the documents issued by guardianship and custody body, confirming the fact of purpose of guardianship - in case of the request for insurance payment concerning the minor children of the injured (supporter) who were for date of death in its dependence or the children who were born after his death;

the reference issued in accordance with the legislation of the Russian Federation in the field of social protection of disabled people, confirming the disability establishment fact - in case of the request for insurance payment concerning the disabled people who were for date of death dependent on the injured (supporter);

the reference of the organization performing educational activities on confirmation of the fact of training in this organization - in case of the request for insurance payment concerning the studying person being for date of death dependent on the injured (supporter);

the conclusion of federal institution of medico-social examination or the medical organization about recognition needing for health reasons foreign leaving - in case of the request for insurance payment concerning the person which was at the time of loss occurrence dependent on the injured (supporter) and needing foreign leaving;

the reference of the body performing appointment and pension payment to the disabled citizen, the certificate of employment service, the conclusion of the medical organization that one of parents, the spouse or other member of the family of the injured (supporter) does not work and it is busy with care of his relatives, - in case of the request for insurance payment concerning the unemployed family members who were at the time of loss occurrence dependent on the injured (supporter), occupied with care of his relatives.

3.2. In the absence of persons having the right according to the civil legislation of the Russian Federation to indemnification in connection with the death of the supporter the spouse, parents, children of the dead, citizens at whom the victim was in dependence if he had no independent income, submit to the insurer the documents provided by subitem 3.1 of this Item.

4. In case of damnification to health the beneficiary submits to the insurer the documents specified in Item of 1 this Instruction and also the following documents.

4.1. The documents issued and which are drawn up according to the procedure established by the legislation of the Russian Federation in the sphere of protection of public health the medical organization to which it was brought or addressed independently injured, irrespective of its form of business, with indication of nature the sustained victim of injuries and injuries, the diagnosis and the period of disability.
