About approval of the Procedure for purpose of technical means of rehabilitation according to functionality of person with disability, the child with disability, injured with anti-terrorist operation

According to the paragraph to the fourteenth Item 44 of the Procedure for ensuring rehabilitation of persons with technical and other means with disability, children with disability and other separate categories of the population approved by the resolution of the Cabinet of Ministers of Ukraine of April 05, 2012 No. 321 (with changes), I order:

1. Approve the Procedure for purpose of technical means of rehabilitation according to functionality of person with disability, the child with disability, injured with anti-terrorist operation which is applied.

2. To department of social protection of persons with disability (Polyakova A. Yu.) provide in the procedure established by the legislation provision of this order on state registration in the Ministry of Justice of Ukraine.

3. To impose control of execution of this order on the deputy minister Fedorovich N. V.

4. This order becomes effective from the date of its official publication.

Procedure for purpose of technical means of rehabilitation according to functionality of person with disability, the child with disability, injured with anti-terrorist operation

I. General provisions

1. This Procedure establishes the mechanism of purpose of technical means of rehabilitation according to functionality of person with disability, the child with disability, injured with anti-terrorist operation (further - the user).

2. In this Procedure terms are used in the values given in the Law of Ukraine "About rehabilitation of disabled people in Ukraine".

3. Providing users is carried out according to the Procedure for ensuring rehabilitation of persons with technical and other means with disability, children with disability and other separate categories of the population approved by the resolution of the Cabinet of Ministers of Ukraine of April 05, 2012 No. 321.

4. Purpose of technical means of rehabilitation is carried out based on the recommendations specified in the individual program of rehabilitation or the conclusion of VKK or the decision of VVK of the user and is obligatory when providing with them.

To users technical means of rehabilitation which functions and equipping correspond to their functionality are appointed.

II. Purpose of means for movement

1. Purpose of chairs wheel is carried out by the doctor or reabilitology by results of assessment of functionality of the user by questioning in the form given in appendix 1 to this Procedure.

In case of assessment of functionality of users for purpose of chairs wheel primary and secondary providing according to appendix 2 to this Procedure is considered.

2. In the presence at the user of one chair wheel or the choice of two chairs by it wheel appointment is carried out on analysis results of compatibility of functions and technical characteristics of such means according to appendix 3 to this Procedure.

3. For each chair wheel obligatory functions, obligatory equipping, additional special equipment depending on individual needs of the user, its wishes concerning options, and also recommendations and contraindications concerning its appointment are established.

4. To users chairs wheel the following types are appointed:

1) wheelchair - means for movement of the user by means of the accompanying person. Is characterized by limited opportunities in positioning of the user (lack of possibility of regulation of system of support of body), it is equipped with handles and it can be equipped with the brake system for its use by the accompanying person.

In the presence at the user of one chair wheel or in case of the choice of two chairs by it wheel wheelchairs are appointed together with multipurpose chairs wheel or chairs wheel with the electric drive basic.

Obligatory functions:

management by means of the accompanying person;

the fixed system of support of body;

footboards feet, removable/fixed with folding support;

armrests removable/folding;

the back fixed;

in the put provision does not lose opportunity to slide on wheels.

Obligatory equipment:

support under shins / zadniki-derzhateli.

Additional special equipment (depending on individual needs of the user):

injury, diseases of basin or the lower extremities (without the expressed residual deformations) that makes impossible independent movement without additional support, with amputating defects or diseases of the upper extremities which do not allow to manage independently chair wheel;

general diseases of cardiovascular system, respiratory system, the musculoskeletal device (other systems and bodies at decompensation stages) and the expressed consequences of the postponed diseases excluding independent movement without additional support;

consequences of surgeries, injuries of the lower extremities and belt of the lower extremities (except disteel department of one foot), such as amputating stump (only one lower extremity), false joint, defect of segment with amputating defects or diseases of the upper extremities which do not give the chance independently to manage chair wheel;

the user can independently sit on chair without additional elements of support and does not require special equipment of seat.

Contraindications to appointment:

absolute:

the user cannot independently sit on chair without additional elements of support and requires special equipment of seat;

relative:

availability of the expressed residual phenomena of the postponed injuries and deformations of backbone, basin;

the user plans to use chair wheel more than 3 hours during the day;

the user requires installation of additional options of chair wheel which are not available to group of wheelchairs;

overall dimensions in group of wheelchairs do not conform to user requirements and complicate operation of wheelchair;

2) low-active chair wheel - means for independent movement of the user in the room or on the street and stay in it till 3 o'clock in day. Is characterized by possibility of independent management it by means of both hands or one hand (in case of installation of the device for management of one hand) and lack of individual adaptation of system of seat.

In the presence at the user of one chair wheel or in case of the choice of two chairs by it wheel low-active chairs wheel are appointed together with medium-active, multipurpose, road chairs wheel basic, chairs wheel with the electric drive basic and road.

Obligatory functions:

management by means of the manual driving rims established on wheels;

system of support of body fixed;

the footboards regulated on height, feet, removable/fixed with folding support;

armrests removable/folding;

the back fixed / removable/folding;

in the put provision does not lose opportunity to slide on wheels.

Obligatory equipment:

support under shins / zadniki-derzhateli.

Additional special equipment (depending on individual needs of the user):

pillow on seat;

anticross over device;

hygienic sitting;

the device for management of one hand.

Recommendations concerning appointment:

injury, diseases of backbone or other type of defeat of nervous system which are followed by paresis or paralysis of one or two lower extremities;

injury, diseases of basin or the lower extremities (without the expressed residual deformations) that makes impossible independent movement without additional support;

general diseases of cardiovascular system, respiratory system, the musculoskeletal device (other systems and bodies at stages of compensation and subcompensation) and the expressed consequences of the postponed diseases excluding independent movement without additional support;

consequences of surgeries, injuries of the lower extremities, belts of the lower extremities (except disteel department of one foot), such as amputating stump (only one lower extremity), false joint, defect of segment;

the user can independently sit on chair without additional elements of support and does not require special equipment of seat;

for movement on the street and for overcoming obstacles (by means of the accompanying person).

Contraindications to appointment:

absolute:

essential restriction of mobility of both upper extremities (can be caused by paresis, paralyzes, consequences of surgeries and injuries);

diseases of cardiovascular or pulmonary system in decompensation stage that makes impossible independent movement of the user in chair wheel;

consciousness violation, the expressed violations of behavior, dementia;

the user cannot independently sit on chair without additional elements of support and requires special equipment of seat;

relative:

availability of the expressed residual phenomena of the postponed injuries and deformations of backbone, basin;

the user plans to use chair wheel more than 3 hours during the day;

the user requires installation of additional options of chair wheel which are not available to group of low-active chairs wheel;

overall dimensions in group of low-active chairs wheel do not conform to user requirements and complicate operation of chair wheel;

3) medium-active chair wheel - means for independent movement of the user in the room and on the street and stay in it more than 3 hours a day. Is characterized by possibility of independent management it by means of both hands or one hand (in case of installation of the device for management of one hand), individual setup of system of support of body, wheel base and/or seat, additional options.

In the presence at the user of one chair wheel or in case of the choice of two chairs by it wheel medium-active chairs wheel are appointed together with low-active, active, road chairs wheel, electroscooters, chairs wheel with the electric drive basic and the increased reliability and functionality, boards for movement.

Obligatory functions:

management by means of the manual driving rims established on wheels;

the system of support of body which is fixed/regulated on inclination corner / regulated on height;
