1. Approve the Inspection routine of the issued technical and other means of rehabilitation which supply persons with disability, children with disability, other persons which is applied.

2. To provide to department of social protection of persons with disability (A. Polyakov) in accordance with the established procedure provision of this order on state registration in the Ministry of Justice of Ukraine.

3. To impose control of execution of this order on the deputy minister N. Fedorovich.

4. This order becomes effective from the date of its official publication.

Inspection routine of the issued technical and other means of rehabilitation which supply persons with disability, children with disability, other persons

1. This Procedure determines the mechanism of carrying out survey of technical and other means of rehabilitation (further - TSR) which free of charge provide persons with disability, children with disability and other separate categories of the population (further applicants) at the expense of public funds according to the Procedure for ensuring rehabilitation of persons with technical and other means with disability, children with disability and other separate categories of the population approved by the resolution of the Cabinet of Ministers of Ukraine of April 05, 2012 No. 321 (with changes) (further - the Procedure).

2. Survey of TSR issued to applicants is procedure for establishment of their compliance of information concerning these products brought in the Centralized databank on disability problems (further - TsBI).

TSR is examined:

according to the address of the applicant and/or his legal (authorized) representative in Minsotspolitiki concerning quality of the received product;

in case of the appeal of the applicant and/or his legal (authorized) representative to republican executive body concerning social protection of the population of the Autonomous Republic of Crimea, structural divisions of social protection of the population of the regional, Kiev and Sevastopol city public administrations, structural divisions of social protection of the population Kiev and Sevastopol of public administrations, executive bodies, district, district in, city, district in the cities (except of Kiev and Sevastopol) councils (daleeorgana of social protection of the population) concerning early replacement or repair of TSR.

3. Inspection of TSR on compliance to the characteristics declared in TsBI is performed by group on carrying out survey of the issued TSR (further - Group) as a part of representatives of social security authorities of the population, Social Security Fund of disabled people (further - Fund) or its territorial departments.

Groups are involved the public representatives (in coordination) and other specialists (experts) in the sphere of medicine and prosthetics (in coordination) including in the videoconference mode.

The members of Group affirm as the corresponding administrative and territorial unit Minsotspolitiki's order.

The group carries out departure for survey, previously having approved time and the place with the applicant and/or his legal (authorized) representative.

By results of survey the corresponding inspection report of the issued technical and other means of rehabilitation which supply persons with disability, children with disability, other persons (further - the Act) in form according to appendix to this Procedure is drawn up.

4. In case of survey of TSR:

specific characteristics of TSR (the name, model, the code, code, cost, etc.) received by the applicant are specified;

technical condition of TSR is described;

in case of availability of claims on functioning of TSR (other shortcomings) by the applicant and/or his legal (authorized) representative the corresponding problems of functioning or shortcomings of this TSR are specified;

photographing, video filming of TSR is carried out;

compliance of TSR and components to the characteristics specified by the producer, condition of depreciation of TSR and components to it are established.

5. If in case of survey the fact of absence of TSR at the applicant is determined, then they and/or his legal (authorized) representative are given explanations about it.

Explanations are entered in the inspection report in which it is noted that inspection of the issued TSR is not performed in connection with its absence at the applicant.

If the applicant or his legal (authorized) representative refuses to provide explanations concerning the fact of absence of TSR, it is noted in the inspection report.

6. In case of absence of the applicant the statement (in any form) about its absence at the actual place of residence (stay) is drawn up.

The head Gruppy not later than in three working days from the date of creation of the act, the applicant and/or his legal (authorized) representative informs the letter with the notification on need of immediate informing on time and the venue of survey of the received TSR.

After obtaining from the applicant and/or his legal (authorized) representative of information on time and the place of survey of the received TSR by Group in the approved time inspection according to requirements of Items 3 - 5 presents About is performed.

If the applicant and/or his legal (authorized) representative from the date of receipt of the letter did not provide in a month information on time and the place of survey of TSR, the head Gruppy not later than in ten working days from the date of the expiration of monthly term informs on this Minsotspolitiki and the corresponding mailing in the personal record of the applicant attaches.

7. The statement of survey is drawn up in triplicate and signed by all members of Group.

One copy of the inspection report is provided to the applicant and/or his legal (authorized) representative that he confirms with the personal signature in the second and third copies, which not later than in five working days after signing go the head Gruppy to Minsotspolitiki and Fund for acceptance of adequate measures.

The fact of refusal of the applicant and/or his legal (authorized) representative to certify receipt of copy of the inspection report is noted by the personal signature in all copies of the inspection report.
