1. Approve enclosed:

form of the reporting No. 5-FK (annual) "Report of children's and youth sports school (specialized Children's and Youth Sports School of Olympic Reserve)";

Instruction for filling of form of the reporting No. 5-FK (annual) "Report of children's and youth sports school (specialized Children's and Youth Sports School of Olympic Reserve)";

form of the reporting No. 5-FK (summary) (annual) "The summary report on activities of children's and youth sports schools (specialized Children's and Youth Sports Schools of Olympic Reserve)";

2. Determine that operation of this order extends on:

1) children's and youth sports schools (children's and youth sports schools in sports, complex children's and youth sports schools, specialized Children's and Youth Sports Schools of Olympic Reserve, children's and youth sports schools for disabled people, specialized children's and youth sports schools for disabled people of Paralympic and deflimpiysky reserve) irrespective of their subordination and pattern of ownership;

2) the Ministry of Education and Science, youth and sport of the Autonomous Republic of Crimea, structural divisions on physical culture and sport of the regional, Kiev and Sevastopol city public administrations;

3) All-Ukrainian sports societies "Dynamo", "Kolos", "Spartak", Ukraine and their cells in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol;

4) Committee on physical training and sport of the Ministry of Education and Science of Ukraine and its branches (Crimean republican, regional, Kiev and Sevastopol city departments on physical training and sport);

5) Ukrainian center for physical culture and sport of disabled people Invasport.

3. 4. To provide to department of the Olympic sport submission in accordance with the established procedure of this order on state registration in the Ministry of Justice of Ukraine.

Declare invalid the order of the Ministry of Ukraine for family, youth and sport of June 5, 2006 No. 1861 "About approval of forms of the reporting No. 5-FK, No. 5-FK (summary) about activities of children's and youth sports schools, specialized Children's and Youth Sports Schools of Olympic Reserve and Instructions for their filling", registered in the Ministry of Justice of Ukraine on June 23, 2006 at No. 741/12615.

I. General provisions

1.
