About approval of the Procedure for creation and representation of requests for information which manager is Public service of Ukraine on safety on transport, and forms for representation of such requests

1. Approve enclosed:

1) the Procedure for creation and representation of requests for information which manager is Public service of Ukraine on safety on transport;

2) request form on information which manager is Public service of Ukraine on safety on transport.

2. To provide to public service of Ukraine on safety on transport:

1) provision of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure;

2) promulgation of this order on the official site of the Ministry of infrastructure of Ukraine.

3. Declare invalid the order of the Ministry of infrastructure of Ukraine of February 26, 2013 No. 114 "About approval of the Procedure for creation and representation of requests for receipt of public information which manager is the State inspectorate of Ukraine for safety on the land transport", registered in the Ministry of Justice of Ukraine on March 14, 2013 at No. 416/22948.

4. This order becomes effective from the date of its official publication.

Procedure for creation and representation of requests for information which manager is Public service of Ukraine on safety on transport

1. This Procedure No. 583 "Questions of execution of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies is developed according to the Law of Ukraine "About access to public information" and the resolution of the Cabinet of Ministers of Ukraine of May 25, 2011".

2. In this Procedure terms are used in the values given in the Law of Ukraine "About access to public information" and the resolution of the Cabinet of Ministers of Ukraine of May 25, 2011 No. 583 "Questions of execution of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine central and local executive bodies".

3. The request for information which manager is Public service of Ukraine on safety on transport (further - Ukrtransbezopasnost) moves physical person or legal entity, consolidation of citizens without the status of the legal entity in oral or written form during personal acceptance or by the direction to Ukrtransbezopasnosti by mail, to e-mail, the telefax or by phone.

4. The request for information is sent in any form.

5. The request for information shall contain:

surname, name, middle name (name) requesting, the postal address or the e-mail address, and also number of communicating device (if is);
