statement for renewal of the license (appendix 2);

the statement for issue of the statement from the register of licenses (appendix 3);

statement for issue of the duplicate of the license (copy of the license) (appendix 4);

the statement for the termination of the licensed type of activity (appendix 5);

the notification on need of elimination of the revealed violations and (or) submission of missing documents (appendix 6);

notification on provision of the license (appendix 7);

the notification on renewal (about refusal in renewal) licenses (appendix 8);

the notification on refusal in provision of the license (appendix 9);

notification on suspension of action of the license (appendix 10);

cancellation notice of the license (appendix 11);

Declare invalid the order of Federal Service for Hydrometeorology and Environmental Monitoring of November 29, 2011 No. 634 "About approval of the document forms used by Federal Service for Hydrometeorology and Environmental Monitoring in the course of licensing according to the Federal Law "About Licensing of Separate Types of Activity" (registration No. 22867) is registered by the Ministry of Justice of the Russian Federation on December 30, 2011.
