1. Approve:

1) form of the announcement of carrying out purchase of goods, pharmaceutical services by method of carrying out the tender according to appendix 1 to this order;

2) the commitment form on participation in the tender according to appendix 2 to this order;

3) form of the list of documents, attached to the request of the potential supplier according to appendix 3 to this order;

4) form of the certificate of lack of overdue debt according to appendix 4 to this order;

5) form of data on qualification according to appendix 5 to this order;

6) form of the table of the prices of the potential supplier according to appendix 6 to this order;

7) form of the bank guarantee according to appendix 7 to this order;

8) form of the bank guarantee (type of providing the tender) according to appendix 8 to this order;

9) form of the standard agreement of purchase according to appendix 9 to this order;

10) form of the standard agreement on rendering pharmaceutical services according to appendix 10 to this order;

18) the commitment form on participation in tender on the conclusion of long-term agreements of delivery of medical equipment according to appendix 18 to this order;

19) form of the standard long-term agreement of delivery of medical equipment according to appendix 19 to this order;

20) form of the supplementary agreement of delivery of the medical equipment which is purchased within the long-term agreement of delivery according to appendix 20 to this order;

21) report form about results of purchase of medicines, preventive (immunobiological, diagnostic, disinfecting) medicines, products of medical appointment according to appendix 21 to this order;

22) report form on monitoring of purchase and deliveries of the medical equipment acquired at the expense of means of the republican budget and also according to the list of the single distributor according to appendix 22 to this order.

2. To provide to committee on control of pharmaceutical and medical activities of the Ministry of health and social development of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days from the date of state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction it the copy in printing and electronic type on official publication in periodic printing editions and in the Republican state company on the right of economic maintaining "The republican center of legal information of the Ministry of Justice of the Republic of Kazakhstan" for placement in Reference control bank of regulatory legal acts of the Republic of Kazakhstan;

3) placement of this order on official Internet resource of the Ministry of health and social development of the Republic of Kazakhstan;

4) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of health and social development of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2) and 3) of this Item.

3. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Tsoi A. V.

4. This order becomes effective from the date of state registration.
