According to Articles 1, 16 Laws of Ukraine "About the state statistics" and Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of June 17, 2015 No. 423 (with changes), for the purpose of reduction of regulatory legal acts in compliance with requirements of the legislation, enhancement of monitoring of provision of social services by subjects which provide social services, order:

1. Approve enclosed:

1) reporting No. 12-sots form (annual) "The report on the organization of provision of social services" (further - form No. 12-sots);

2) Instruction for filling of form of the reporting No. 12-sots (annual) "Report on the organization of provision of social services".

2. And to provision of social services (O. Sulim) to provide to management for elderly people submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. To executive body concerning social protection of the population of the Autonomous Republic of Crimea, to structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations to provide collection, control, processing and submission of information on form No. 12-sots to the Ministry of social policy of Ukraine till February 10.

4. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of January 24, 2011 No. 20 "About approval of form of the reporting No. 12-sots (annual) "The report on the organization of social servicing (provision of social services) by the territorial centers of social servicing (provision of social services)" and the Instruction for its filling", registered in the Ministry of Justice of Ukraine on February 09, 2011 on No. 169/18907.

5. This order becomes effective from the date of its official publication.

6. To impose control of execution of this order on the deputy minister N. Fedorovich.

Instruction for filling of form of the reporting No. 12-sots (annual) "Report on the organization of provision of social services"

I. General provisions

1. This Instruction determines procedure for filling of form of the reporting No. 12sots (annual) "The report on the organization of provision of social services" (further - the Report) which contains the indicators characterizing work of subjects which provide social services (the territorial centers of social servicing (provision of social services) the centers of provision of social services, structural divisions concerning provision of social services of executive bodies of the integrated territorial communities) (further - the subjects providing social services), about provision of social services to citizens who are in difficult vital circumstances, including to women, including citizens of advanced age, persons with disability, patients (from among persons of working-age for the period before establishment of group of disability by it, but no more than four months), which are not capable to self-service and need permanent assistance, to the citizens who are in difficult life situation in connection with unemployment (and are registered in public service of employment as looking for work), natural disaster, catastrophic crash (and have in the dependence minor children, children with disability, elderly people, persons with disability) if the average monthly comprehensive income of their families is lower, than subsistence minimum for family.

2. The report is constituted once a year by the subjects providing social services and moves till January 20 of the year following for reporting, to structural division concerning social protection of the population of local public administration or to executive body of city, district council in the city, the integrated territorial community.

3. The structural division concerning social protection of the population of local public administration or executive body of city, district council in the city, the integrated territorial community submits the Report till January 25 of the year following for reporting, to executive body concerning social protection of the population of the Autonomous Republic of Crimea, to structural division concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations.

4. Executive body concerning social protection of the population of the Autonomous Republic of Crimea, structural divisions concerning social protection of the population of the regional, Kiev and Sevastopol city public administrations submit the Report till February 10 of the year following for reporting, to the Ministry of social policy of Ukraine and head departments of statistics in the Autonomous Republic of Crimea, areas, the city of Kiev and to Management of statistics in the city of Sevastopol.

5. The ministry of social policy of Ukraine the year following reporting submits on March 15 to Public service of statistics of Ukraine the general report across Ukraine in general and on regions.
