About approval of the Procedure for provision and contents of the reporting by governing bodies of gambling zones

1. Approve the enclosed Procedure for provision and contents of the reporting by governing bodies of gambling zones.

Declare invalid the order of the Ministry of Finance of the Russian Federation of May 25, 2007 No. 45n "About the Procedure for provision and contents of the reporting by governing bodies of gambling zones" (registration No. 9665) is registered in the Ministry of Justice of the Russian Federation on June 19, 2007.

Procedure for provision and contents of the reporting by governing bodies of gambling zones

This Procedure establishes rules of provision of the reporting by governing bodies of gambling zones, and also requirements imposed to contents of the reporting.

The reporting is provided by governing bodies of gambling zones in the Ministry of Finance of the Russian Federation and tax authorities on installation site of game table and (or) the gaming machine (the location of collection point for rates of bookmaker office or collection point for rates of totalizator, processing center of bookmaker office or processing center of totalizator) (further - tax authorities).

The reporting is provided monthly as of the last day of month under report in time no later than 15 calendar days after the term of submission of data by the organizer of gamblings in governing body of gambling zone, stipulated in Item 4 Rules of representation by the organizer of gamblings of the data necessary for control of compliance with law about state regulation of activities for the organization and carrying out the gamblings approved by the order of the Government of the Russian Federation of August 23, 2007 No. 540 (The Russian Federation Code, 2007, No. 35, Art. 4323; 2010, No. 49, Art. 6525; 2013, No. 16, Art. 1951; No. 51, Art. 6882; 2015, No. 28, Art. 4225; No. 48, Art. 6840; 2016, No. 28, Art. 4747).

The reporting is constituted on recommended sample according to appendix to this Procedure.

In the reporting the following information is specified:

in column 1 - sequence numbers;

in column 2 - full name of the organizer of gamblings, number and date of the operating authority on the organization and carrying out gamblings in gambling zone;

in column 3 - net assets value of the organizer of gamblings;

in column 4 - land area, located in the gambling zones leased to organizers of gamblings;

in column 5 - land area, located in the gambling zones transferred to the possession to organizers of gamblings;

in columns 6, 7, 8, 9, 10, 11 - quantity of gambling institutions in which activities for the organization and carrying out gamblings (including branches or other places of implementation of activities for the organization and carrying out gamblings) this organizer of gamblings are performed;

in columns 12 and 13 - the number of the gaming equipment used for carrying out gamblings by this organizer of gamblings;

in column 14 - the actual number of workers of the organizer of gamblings;

in column 15 - the number of visitors of gambling institution for the accounting period;

in columns 16, 17, 18 - quantity of objects where the services accompanying gamblings are rendered;

in column 19 - the name of lottery, and also the name of the operator of the extended lottery;

in column 20 - number and date of the decision on cancellation of the operating authority on the organization and carrying out gamblings in gambling zone (in the presence).

When filling the reporting only one indicator fits into every line and the columns corresponding to it. In case of lack of any indicators in line and the corresponding column the crossed out section is put.

The reporting goes to the Ministry of Finance of the Russian Federation and tax authorities on paper or in electronic form with use of information and telecommunication networks public, including Internet networks.

The reporting provided on paper is signed by the head of governing body of gambling zone (person, the acting as the head) and certified by seal (in the presence of seal).

The reporting provided in electronic form is signed by the strengthened gambling zone (person, the acting as the head) qualified by the digital signature the head of governing body.
