About forms of certificates of the salary for award of pension by the government employee

According to Items 2, of 10, of the 12th Section XI "Final and transitional provisions" of the Law of Ukraine "About public service", to Items 9, 10 Regulations on the Pension fund of Ukraine, the Cabinet of Ministers of Ukraine approved by the resolution of July 23, 2014 No. 280, and to Item 5 of the Procedure for the purpose of pensions to some categories of persons approved by the resolution of the Cabinet of Ministers of Ukraine of September 14, 2016 No. 622, board of the Pension fund of Ukraine decides:

1. Approve forms of certificates of the salary for appointment according to Items 10 and 12 of the Section XI "Final and transitional provisions" of the Law of Ukraine of December 10, 2015 No. 889-VIII "About public service" pensions according to article 37 of the Law of Ukraine of December 16, 1993 No. 3723-XII "About public service" which are applied:

form of the certificate of salary components for award of pension of the government employee (official pay rate, the allowance for rank, length-of-service allowance);

form of the certificate of salary components for award of pension of the government employee (for any 60 calendar months of work in a row before the request for pension);

form of the certificate of salary components for award of pension to person having at least 20 years of length of service on the positions referred to categories of positions of public service and which at the time of retirement does not hold position of public service.

2. Recognize invalid resolutions of board of the Pension fund of Ukraine:

of February 14, 2011 No. 5-1 "About accomplishment of the decision of District administrative court of the city of Kiev in the matter of No. 2a-4753/09/2670", registered in the Ministry of Justice of Ukraine on February 18, 2011 for No. 210/18948;

of September 4, 2013 No. 15-1 "About accomplishment of decisions of District administrative court of the city of Kiev for No. 2a-8893/12/2670, No. 2a-4753/09/2670", registered in the Ministry of Justice of Ukraine on September 26, 2013 for No. 1661/24193;

of July 4, 2016 No. 15-2 "About introduction of amendments to the resolution of board of the Pension fund of Ukraine of September 4, 2013 No. 15-1", registered in the Ministry of Justice of Ukraine on July 25, 2016 for No. 1020/29150.

3. To legal management (Ryabtseva T. B.) submit this resolution on state registration to the Ministry of Justice of Ukraine.

4. This resolution becomes effective from the date of its official publication.
