1. Approve:

1) the commitment form on participation in tender according to appendix 1 to this order;

2) form of the list of documents, attached to the request of the potential supplier, according to appendix 2 to this order;

3) form of the certificate of lack of overdue debt on all obligation types of the potential supplier according to appendix 3 to this order;

4) form of data on availability and the number of specialists with indication of their qualification, years of service of occupational works according to appendix 4 to this order;

5) form of data on the qualification confirming work experience in the pharmaceutical market according to appendix 5 to this order;

6) form of the price offer according to appendix 6 to this order;

7) form of the bank guarantee (type of providing the bid) according to appendix 7 to this order;

8) form of the long-term agreement according to appendix 8 to this order;

9) form of the supplementary agreement according to appendix 9 to this order;

10) form of the bank guarantee according to appendix 10 to this order.

2. Pharmaceutical and copper of activities of the Ministry of health and social development of the Republic of Kazakhstan in the procedure established by the legislation to provide to committee on control:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days from the date of state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction it the copy in printing and electronic type on official publication in periodic printing editions and in the Republican state company on the right of economic maintaining "The republican center of legal information of the Ministry of Justice of the Republic of Kazakhstan" for placement in Reference control bank of regulatory legal acts of the Republic of Kazakhstan;

3) placement of this order on official Internet resource of the Ministry of health and social development of the Republic of Kazakhstan;

4) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service of the Ministry of health and social development of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2) and 3) of this Item.

3. To impose control of execution of this order on the vice-Minister of health and social development of the Republic of Kazakhstan Tsoi A. V.

4. This order becomes effective from the date of state registration.
