1. Approve the enclosed Rules of issue of veterinary documents and requirements to their forms.

2. To provide to department of veterinary and food safety of the Ministry of Agriculture of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan the direction it the copy on official publication in periodic printing editions and in information system of law of Ad_let;

3) placement of this order on Internet resource of the Ministry of Agriculture of the Republic of Kazakhstan and the intranet portal of state bodies.

3. To impose control of execution of this order on the supervising vice-Minister of Agriculture of the Republic of Kazakhstan.

4. This order becomes effective after twenty one calendar days after day of its first official publication.

Rules of issue of veterinary documents and requirements to their forms

Chapter 1. General provisions

1. These rules of issue of veterinary documents and requirements to their forms (further - Rules) are developed according to subitem 46-4) of article 8 of the Law of the Republic of Kazakhstan of July 10, 2002 "About veterinary science" (further - the Law), and also according to the subitem 9) of Item 1 of article 9 of the Law of the Republic of Kazakhstan of July 9, 2004 "About protection, reproduction and use of fauna", and determine procedure for issue of veterinary documents and requirements to their forms.

2. In Rules the following concepts are used:

1) the breeding certificate - the document confirming family tree, productive and other qualities of breeding products (material), issued by the breeding plant, breeding economy, the breeding center, the distribyyuterny center for realization of seed of breeding animals, the breeding loud-speaker and republican chamber on breeds of cattle performing activities in the field of breeding livestock production;

2) territorial subdivisions of department (further - territorial subdivisions) - the territorial subdivisions located on the corresponding administrative and territorial units (the area, the city of republican value, the capital, the area, the city of regional value);

3) department of authorized body in the field of veterinary science (further - department) - Committee of veterinary control and supervision;

4) animals - all types of the farm animals and birds directly related to agricultural production cultivated by the person, and also the wild animals (mammals, birds, reptiles, Amphibia, fishes, mollusks, insects and others) who are in condition of natural freedom on the land in water, the atmosphere and the soil;

5) divisions of production supervision - the created administration of the objects of production performing slaughter of animals, conversion and sales of products and raw materials of animal origin, division by determination of compliance of animals, products and raw materials of animal origin to veterinary standard rates;

6) the applicant - the physical person or legal entity which submitted the application for receipt of the veterinary document.

3. Forms of the veterinary certificate, the veterinary and sanitary conclusion, the veterinary reference are issued in the state or Russian languages, the veterinary certificate - if necessary in English.
