1. Approve:

1) form of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance which is applied;

2) the Procedure for issue of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance which is applied.

2. To department of social protection of disabled people (Polyakova A. Yu.) provide submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

3. I reserve control over the implementation of this order.

4. This order becomes effective from the date of its official publication.

Procedure for issue of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance

1. This Procedure determines the mechanism of issue of the reference for receipt of privileges by disabled people who have no pension entitlements or the public assistance, according to the Laws of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", "About the government public assistance to persons who have no pension entitlement, and to disabled people" (further - the Reference).

2. The reference is the document which grants the right to the disabled people who do not have pension entitlements or the public assistance to use the privileges established by the Law of Ukraine "About bases of social security of disabled people in Ukraine" and other acts of the legislation.

3. The reference is issued by structural divisions of social protection of the population of district, district public administrations in the cities of Kiev and Sevastopol, executive bodies of city councils at the place of residence to the disabled people who do not have pension entitlements or the public assistance or their legal representatives.

4. The reference is issued free of charge based on the statement of the disabled person or his legal representative on the sample given in appendix to this Procedure.

Are enclosed to the application:

the passport copy of the disabled person (if the legal representative - passport copies of the legal representative and the document confirming him powers addresses);

the copy of the reference to the inspection report medico-social commission of experts in the form given in primary accounting documentation No. 157-1/o "The statement from the inspection report medico-social commission of experts", approved by the order of the Ministry of Health of Ukraine of July 30, 2012 No. 577, registered in the Ministry of Justice of Ukraine on September 5, 2012 at No. 1504/21816;

one color photo of the disabled person the 3,5 x 4,5 size see.

5. The structural division of social protection of the population of district, district public administrations in the cities of Kiev and Sevastopol, local government body to which the statement for provision of the reference arrived checks information on stay on accounting and receipt of pension or the public assistance in bodies of the Pension fund or social protection of the population of the disabled person taking into account data of the automated system of document handling to destination and pension payment, the central storage of databases of the Ministry of social policy of Ukraine.

6. The term of consideration of the application of the disabled person or his legal representative about issue of the Reference or notification of the disabled person on refusal in its provision constitutes five working days from the date of receipt of such statement by structural divisions of social protection of the population of district, district public administrations in the cities of Kiev and Sevastopol, executive bodies of city councils.

7. The bases for refusal in issue of the reference is submission of doubtful data, origin at person of pension entitlement or the public assistance according to the Laws of Ukraine "About the government public assistance to disabled people since the childhood and to handicapped children", "About the government public assistance to persons who have no pension entitlement, both to disabled people" and appointment such pension or the public assistance.

8. The reference is valid for disability establishment term.
