According to article 19 of the Law of Ukraine "About access to public information", to Item 2 of the Presidential decree of Ukraine of May 5, 2011 to No. 547 "Questions of ensuring with executive bodies of access to public information", item 4 of the resolution of the Cabinet of Ministers of Ukraine of May 25, 2011 No. 583 "Questions of execution of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies" I order:

1. Approve:

request form on receipt of the public information which is in ownership of Public service of Ukraine concerning work and its territorial authorities which is applied;

Procedure for creation, giving and consideration of requests for receipt of public information which manager is Public service of Ukraine concerning work and its territorial authorities.

2. Declare invalid the order of the Ministry of social policy of Ukraine of August 13, 2012 No. 499 "About approval of forms of requests for receipt of public information and the Procedure for creation and representation of requests for receipt of public information which manager is the State inspection of Ukraine concerning work", registered in the Ministry of Justice of Ukraine on September 3, 2012 at No. 1480/21792.

3. To provide to management of addresses of citizens and control of execution (Gatsanyuk S. P.) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

4. To public service of Ukraine concerning work () provide to R. T. Chernega placement of request form on receipt of public information which manager is Public service of Ukraine concerning work, on its official site.

5. This order becomes effective from the date of its official publication.

6. To impose control of execution of this order on the first deputy Minister Krentovskaya A. P.

Procedure for creation, giving and consideration of requests for receipt of public information which manager is Public service of Ukraine concerning work and its territorial authorities

I. General provisions

1. This Procedure determines the mechanism of creation and representation of requests for receipt of public information which manager is Public service of Ukraine concerning work and its territorial authorities.

2. In this Procedure terms are used in the values given in the Law of Ukraine "About access to public information".

3. Access to public information which manager is Gostruda and her territorial authorities is provided by provision of information on requests for receipt of public information.

II. Creation and representation of requests for receipt of public information

1. The request for receipt of public information (further - request) is sent Gostruda and to her territorial authorities by physical person or legal entity, associations of citizens without the status of the legal entity, except subjects of powers of authority when implementing of the functions by them, in oral or written form during personal acceptance or by sending by mail, to e-mail, the telefax or by phone.

2. The request can be individual or collective.

3. The written request is sent in any form.

4. The request shall contain:

surname, name, middle name (name) requesting, the postal address or the e-mail address, and also number of the means of communication if that is available;

the general description of information or type, the name, details or contents of the document concerning which the request is made if requesting it it is known;

the signature and date (on condition of submission of the written request).

5.
