1. Approve on representation of the Ministry of Education of the Republic of Belarus enclosed:
1.1. form of the state statistical reporting 4 maintenance costs of children (Minobrazovaniye) "The report on the children recognized being in socially dangerous provision and needing the state protection, and about collection of maintenance costs of the children who are on the state providing" quarter frequency and to enact it starting with the report for January-September, 2016;

Instructions for filling of form of the state statistical reporting 4 maintenance costs of children (Minobrazovaniye) "The report on the children recognized being in socially dangerous provision and needing the state protection, and about collection of maintenance costs of the children who are on the state providing" and to enact them starting with the report for January-September, 2016.

Extend the form of the state statistical reporting specified in subitem 1.1 of Item 1 of this resolution to the child care residential care facilities, organizations of general secondary, professional, secondary vocational and higher education, structural divisions of city, district executive committees, local administrations of areas in the city of Minsk performing state powers of authority in education.
3. Recognize invalid:
