1. Approve:

1) Rules and terms of provision of the report on living conditions, training, education and about the state of health of the adopted child (further - Rules), according to appendix 1 to this order;

2) report form about living conditions, training, education and about the state of health of the adopted child, according to appendix 2 to this order.

2. To committee on protection of the rights of children in the procedure established by the legislation to provide:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) within ten calendar days after state registration of this order the direction of its copies on official publication in the Information system of law of regulatory legal acts of the Republic of Kazakhstan of Ad_let and periodic printing editions on the electronic medium with appendix of the paper copy certified by official stamp;

3) within five working days from the date of receipt of the registered this order the direction it the copy in printing and electronic type, the confirmed seal of the Ministry of Education and Science of the Republic of Kazakhstan and the witnessed digital signature of person, representative to sign this order, for inclusion in Reference control bank of regulatory legal acts of the Republic of Kazakhstan;

4) placement of this order on Internet resource of the Ministry of Education and Science of the Republic of Kazakhstan;

5) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Department of legal service and international cooperation of the Ministry of Education and Science of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1), 2) and 3) of this Item.

3. To impose control of execution of this order on the vice-Minister of Education and Science of the Republic of Kazakhstan B. Asylova.

4. This order becomes effective after ten calendar days after day of its first official publication, except for Items 8, of 9 and 10 Rules which become effective since January 1, 2017.

Rules and terms of provision of the report on living conditions, training, education and about the state of health of the adopted child

Chapter 1. General provisions

1. These rules and terms of provision of the report on living conditions, training, education and about the state of health of the adopted child (further - Rules) are developed according to item 4 of article 86 of the Code of the Republic of Kazakhstan of December 26, 2011 "About scrap (matrimony) and family" (further - the Code).

2. Rules determine procedure and terms of provision of the report on living conditions, training, education and about the state of health of the adopted child (further - the report).

3. In these rules the following determinations are used:

1) agencies on adoption (adoption) - the non-profit, foreign organizations performing activities for adoption (adoption) of children in the territory of the state and accredited for implementation of similar activities in the territory of the Republic of Kazakhstan according to the procedure, established by the Code;

2) the Republican databank of orphan children, children without parental support, and persons wishing to accept children on education in the families (further - the Republican databank) - the database containing the information about orphan children, children without parental support, and also about persons wishing to accept orphan children, children without parental support on education in the families;

Chapter 2. Procedure and terms of provision of the report on living conditions, training, education and about the state of health of the adopted child citizens of the Republic of Kazakhstan, constantly living in the territory of the Republic of Kazakhstan

5. The adoptive parents who are citizens of the Republic of Kazakhstan, constantly living in the territory of the Republic at least once a year before achievement of eighteen-year age by the child provide the report in the body performing functions on guardianship or custody (further - body), in the place of decision of court about adoption of the child.

6. The report is constituted on Kazakh or Russian and is provided in body in one month prior to date of approach of term of delivery of the report and no later than month of the expiration of delivery of the report.

7. Provision of reports by the adoptive parents who are citizens of the Republic of Kazakhstan, constantly living in the territory of the Republic is performed through body or the Republican databank.

8. In case of receipt of the report in body, the employee of body brings electronic report version in the Republican databank.

9. By provision of the report through the Republican databank adoptive parents independently enter private office in the Republican databank and make active the bookmark "Reports".
