Procedure for creation, maintaining and access to data of the Single information database on internally displaced persons

1. This Procedure determines conditions of creation, maintaining and access to data Single the information database about internally displaced persons (further - the database).

2. The database - the automated databank created for ensuring single state accounting of the physical persons which are internally displaced.

The owner of the database and exclusive property rights of intellectual property on its software is the state on behalf of Minsotspolitiki.

The owner of information processed in the database is Minsotspolitiki.

2-1. The database manager is the state company "Data Processing Center of the Ministry of Social Policy of Ukraine" which performs actions for implementation, administration, technical support and support of the software of the database, is responsible for its maintenance, preserving and data protection, saved up in the database, takes technical and technological measures for provision, blocking, cancellation of access to the database, makes search and selection of the analytical reports given for preparation.

Personal data processing and provision of information from the database is provided with the database manager with observance of requirements of the Laws of Ukraine "About information" and "About personal data protection".

3. Information in the database is entered by structural divisions concerning social protection of the population of the district, district state administrations in Kiev, executive bodies city, district in the cities (in case of creation) councils (further - authorized bodies) electronically based on the application submitted by the internally displaced person which form is approved by Minsotspolitiki.

The database allows to process personal data of the registered internally displaced persons according to requirements of the Law of Ukraine "About personal data protection".

If information about internally displaced persons is entered in the database for the first time, unique number of record which is displayed in the help about registration of the internally displaced person (further - the help), according to appendix to the Procedure for registration and issue of the help about registration of the internally displaced person approved by the resolution of the Cabinet of Ministers of Ukraine of October 1, 2014 No. 509 "About accounting of internally displaced persons" is automatically created (The Official Bulletin of Ukraine, 2014, No. 81, Art. 2296; 2015, No. 70, Art. 2312; 2016, No. 46, Art. 1669).

Information on person is entered in the database and helps in Ukrainian and Latin letters according to rules of transliteration. Information on the foreigner or the person without citizenship is entered in Ukrainian and Latin letters according to entries in the identity documents issued by competent authorities of foreign state.

Personal data are stored in the database within ten years after the end of action of the circumstances determined by article 1 of the Law of Ukraine "About providing the rights and freedoms of internally displaced persons".

Personal data are removed or destroyed according to the procedure, established according to requirements of the Law of Ukraine "About personal data protection". Personal data in the database are liable to destruction after the term of their storage.

Destruction of personal data is performed by the method excluding further upgradeability of such data.

4. In the database the following information about internally displaced persons is entered:

general information about person (surname, name, middle name, nationality, birth date and birth place, floor);

details of the documents confirming citizenship of Ukraine, proving the identity or her special status or the certificate of birth of the child (the name of the document, series, number (in case of availability), date of issue and the subject which issued the document);

data on registration number of accounting card of the taxpayer (except physical persons which owing to the religious beliefs refused acceptance of registration number of accounting card of the taxpayer, notified on it the relevant monitoring body and have about it mark in the passport or which are not citizens of Ukraine);

number of the help, date of issue and the name of the body which issued it;

data on the last registered and actual place of residence / stay of the internally displaced person in the territory where there were circumstances specified in article 1 of the Law of Ukraine "About providing the rights and freedoms of internally displaced persons";

data on the actual residence for date of the address;

the address to which with the internally displaced person official correspondence or delivery of official mail, and phone number can be conducted;

data of rather juvenile, minor children who arrived together with the internally displaced person;

data of rather juvenile children who arrived without maintenance by legal representatives;

information about persons who are directly performing care of juvenile children, incapacitated persons or persons whose capacity to act is limited;

information about the representative of guardianship and custody body;

data on housing, social, medical, educational and other requirements of internally displaced persons, condition of their providing;

data on types of social payments which are received by person;

data on the place of study/education of the child (name of educational institution);

data on educational level, specialization in professional education, profession;

data on employment (position, name of the company, organization, organization);

data on availability at person of disability with indication of the reasons of approach, group (subgroup) of disability, nosological signs and the needs for technical and other means of rehabilitation, sanatorium treatment;

data on availability at any of family members on the deposit bank account of money in the amount of exceeding the 25-fold size of the subsistence minimum established for able-bodied persons;
